Complete Your Health Assessment Online Early for a Chance to Win
The State Health Plan is encouraging active and pre-Medicare retiree State Health Plan subscribers to
complete their Health Assessments early and online for a chance to win a $250 Amazon gift card and/or
an Apple Watch. Health Assessments are promoted annually as a component of Open Enrollment.
Subscribers can earn down subscriber monthly health plan premiums (as applicable) by completing their
Health Assessment.
How to Enter:
Complete all four sections of the Health Assessment (HA) in your Personal Health Portal by October 15,
2016. Subscribers must click “Submit Now” in order to submit their HA and automatically be entered in
the drawing(s).*
How to Win:
All subscribers who complete their HA by October 15, 2016 will have the chance to win. Subscribers who
complete their HA between May 1 and October 8, 2016, will have up to six chances to win a $250
Amazon gift card. The earlier a subscriber completes their HA, the more chances they will have to win.
Previous winners of Amazon gift cards will be removed from future Amazon gift card drawings. All
names, irrespective of whether they have won an Amazon gift card, will be entered into the grand prize
drawing for the Apple Watch.

How to Complete Your Health Assessment Online
1. Go to the State Health Plan website, www.shpnc.org
2. Click on the My Personal Health Portal icon
3. Enter your username and password, and click “Submit”
a. If you forgot your username or password, click on “I forgot my username/password”.
b. First time users, click on “Create an Account”
4. Click on >Dashboard
5. Click on Health Assessment
6. Complete all four sections of your Health Assessment:
a. Health History
b. Lifestyle
c. Improving Your Health
d. Additional Question
7. Click “Submit Now”*

*Your Health Assessment will not be considered complete if you select “Save & Finish Later”. You must
click on “Save Now” to complete your Health Assessment and be entered into the drawing(s).
Official Rules and Regulations
To be eligible to win, you must:
 Be an active State Health Plan subscriber,
 Be 18 years of age and older, and
 Complete all four sections of your Health Assessment between May 1 and October 15, 2016.
 State Health Plan employees are not eligible to win.
