I agree to collect and pay required sales tax, which will then be turned over to the Chamber and recorded on their sales
tax license.
The sales tax for the Town of Johnstown is 3.0% and the State of Colorado is 2.9% (5.9% total).
The Town of Milliken sales tax is at 2.5% (5.4% total).
City and State sales tax must be reported and submitted on the State Sales Tax Form. The vendor is responsible for
collecting the sales tax and submitting it to the Town. Please see the attached letter for details.

Fees and Setup for 2014 Farmers’ Market
Fees ($7.50 day) ( $20.00 for a month) ( $80.00 for the season)
Set Up: Spaces will be assigned to each vendor at the beginning of set up on June 7th - Milliken
and August 14th - Johnstown. Set up may begin no sooner than 2:15pm in Milliken or 4:00pm in Johnstown. Vendors
must be set up by 2:45pm in Milliken and by 4:45 pm in Johnstown.
Canopies or tents must be tied down or otherwise secured. All spaces must be cleaned up after tear down. Please see
the Market Director at the Chamber booth prior to setting up for your space assignment.
Milliken on Saturdays, 3 p.m. to 7:00pm
Johnstown on Fridays, 5 p.m. to dusk
Parking lot east of the Milliken Town Hall (1101 Broad St)
Parish Street between Charlotte and W South 1st St.

In consideration of my participation in the Johnstown Milliken Market, I agree with the above information and agree to
operate our booth in a safe and sensible manner. I further agree to comply with all governmental and/or health
department regulations pertaining to the sale of the products I offer. I understand and agree that any infraction which is
not immediately corrected may bar me from further participation in the current or future market dates. I agree to hold
harmless the Johnstown-Milliken Chamber of Commerce (JMCOC) and/or the Town of Johnstown and/or the Town of
Milliken from any action, damage or injury which I may sustain while participating in the Johnstown Milliken Farmer's
Market.
The JMCOC reserves the right to cancel the Farmer's Market due to Acts of God, weather or other unforeseen events
that may jeopardize the health and safety of the public.
