IMPORTANT REMINDER: If you have depicted individuals other than yourself, you have one more step in
order to complete your entry. Each individual in your video other than yourself will need to complete and fax a
consent form. You will receive a link to the consent form with all the details once you successfully submit your
video. Your entry will not be judged until we receive consent forms from each person depicted in your video
other than yourself.
