Being the parent or legal guardian of the listed minor child, I have read and agree to be bound by the terms of this agreement. The listed child desires to play football with South Dakota
Junior Football. It is fully understood that no professional coaches are available to coach this league, therefore this task must be performed by the parents and others volunteering without
compensation to assist the participants in organizing and directing their play. All parents are invited to participate in this task. Each parent is solely responsible for their child’s participation
and is expected to monitor their play and physical condition at all times without expectation or assumption of this task by any other individual. Monitoring the fitting, use and return of issued
equipment is the sole responsibility of the parents and not of anyone associated with the team or league. A $250 replacement fee will be charged for equipment not returned. Parents are
required to know and abide by league rules which are available through South Dakota Junior Football. Parents at any point in time may immediately withdraw their child from participation.
Those designated as coaches simply agree to help assist in play selection, position assignments and parents representation for league and scheduling communications. Those
designated as coaches have no special training or expertise in the performance of this task. Their knowledge is purely that of a spectator and interested person and therefore make
no such representation that they have any special expertise in coaching football or the prevention or treatment of injuries.
It is understood that football is a contact sport with an inherent risk of serious injury to the participant, whether this risk may be caused by actions or inactions of the participant or others,
by weather or other conditions in which the activity takes place, or by the negligence of a coach, official, South Dakota Junior Football or its officers or directors. On behalf of myself and
the child participant, I agree to assume the risk associated with participation in this football program and hereby release, discharge and covenant not to sue any other participant, parent,
coach, official, South Dakota Junior Football, its officers or directors, and all landowners, agents or lessees who have granted permission to use the land and fields where the league
games are played or practices are held, from any and all liability, claims, demands, losses, or damages caused or alleged to be caused in whole or in part by the negligence of the
released parties or otherwise resulting from participation in the football program. By signing below, I acknowledge that specific educational materials on concussions are available for my
review at the web site of South Dakota Junior Football at www.sdjuniorfootball.com and that I will review them.
I hereby grant consent to any and all health care providers designated by South Dakota Junior Football to provide the child any necessary medical care as a result of any injury/illness.
This consent includes first aid and transportation to/from health care providers. It is understood that South Dakota Junior Football has no medical insurance that would cover participants.
Responsibility for any health care costs and any form of insurance rests solely upon the participants and their parents/guardians.
In consideration of the child being allowed to participate in any way in South Dakota Junior Football related events and activities, I agree that such participant’s likeness may be
photographed or videotaped and that such image may be published in an outlet used to promote or publicize this football program.
Any questions concerning these matters should be directed in writing to South Dakota Junior Football directly. No coach claims expertise in this area or assumes any responsibility for
these matters. The Board of Directors of South Dakota Junior Football Inc reserves the right to not accept, decline or revoke any player registration at any time.
I have carefully read this agreement and have understood all of the language and terms thereof. I agree to be bound by all the terms of this agreement.
