REQUEST FOR QUOTATION
Quotation is carried out in accordance with the principles of fair competitiveness, publicness
and transparency. The Public Procurement Law of January 29, 2004 – consolidated text of 2013
(Dz. U. – Journal of Laws of 2013 no. 907 as amended) does not apply to this request for
quotation. The request relates to the project “Development of anticancer targeted therapy based
on small molecules regulating mutated RAS protein G12V” that is co-financed by The
Operational Programme Smart Growth - Measure 1.2. According to the principle of
competitiveness defined in Guidelines for eligibility of expenses dedicated to projects
submitted to competition under Measure 1.2., Operational Programme – Smart Growth 2015,
Personather Ltd. hereby requests to submit price offer for:

The quality of the reagents offered should be the highest, and the test report will be required to confirm
the quality and structure of the molecules.
In addition, double the 30-day delivery deadline will result in the automatic termination of the contract,
and in the event of grave delays affecting the execution of the research activities may also result in
exclusion from future supply proceedings under the project.
PARTICIPATION TERMS
The procedure is dedicated for Bidders meeting the following requirements:
a) are able to operate in the area matching the offer subject
b) are qualified, experienced and have all facilities and staff resources required to execute
the order with the highest standard of service
c) are economically and financially able to execute the order
Bidders1 having any capital connection or personal relation with the Purchaser shall
be excluded from the procedure.
The abovementioned conditions should be met by all the members of the consortium
or any other association that consists of more than one entity, willing to submit an offer
in the procedure.
Fulfillment of the abovementioned conditions should be declared in an appropriate statement.
If the Bidder’s financial/legal situation is questionable, the Purchaser is entitled to ask all of the
chosen Bidders to present documents confirming fulfillment of the abovementioned
requirements.

a) participation in the company as a partner or a partner in a partnership;
b) owning at least 10% of stocks and shares;
c) being a member of the supervisory or management authority, being a proxy;
d) remaining married, in consanguinity or affinity in a straight line, consanguinity or affinity of the second degree
in the collateral line, by adoption, under care or guardianship.
The score will be calculated according to the formula: S = C + T, wherein:

The Purchaser is entitled to choose no Bidder within the procedure.
FORMAL REQUIREMENTS OF THE OFFER
The offer must include:

Bidder’s company full name, address and telephone number;
Company’s stamp, date of offer compilation and Bidder’s signature;
Total value of the offer including prices of each element specified in the request,
however, a Bidder is not required to offer all of the chemical compounds from the
quotation;
5) Declaration of meeting the participation terms in the procedure.
Partial offers are allowed. Conditional offers are not allowed.

Offers submitted after deadline will be considered invalid. The data and hour of offer reception
is definitive.
The Bidder is entitled to modify or withdraw the offer before deadline. The Purchaser
is allowed to ask the Bidder to explain the offer details.
The offer comparison result is definitive and the overall result of tender procedure
is unappealable.

ADDITIONAL INFORMATION
The choice of the most favorable offer will be sent in electronic form to the e-mail address
indicated in the offer.
The Purchaser is entitled to conclude a supplementary contract with the selected Bidder,
provided that its value does not exceed 50% of the main contract. The subject
of the supplementary contract will comply with the subject of the main contract.
The Purchaser is entitled to modify the substance of the offer. If the modifications affect
the content of submitted offers, the deadline for submission will be postponed.
The Purchaser is entitled to annul the procedure without justification, modify the number
of ordered goods, as well as to choose no offer within the procedure.
