2016/17
Schools and districts with exemplary and promising practices in the area of parent and family
engagement are encouraged to submit an Exemplary and Promising Practices in Parent
Engagement summary. The summary should highlight a practice that shows leadership,
teamwork, the use of Action Plans, innovation, outreach, and evaluation. Selected summaries
may be included in the annual edition of the Exemplary and Promising Practices in Parent
Engagement document.
Complete the following information and include this cover page and a copy of your Action Plan
(blank Action Plan form included with this form) when you submit your school’s or district’s
exemplary practice. Text boxes on the application form will expand to accommodate content. Please
limit application to three pages, not including this cover sheet, Action Plan, or supporting
document(s).
