Internet and Computer User Agreement
SY2011-2012
1. All computer users must sign an "Internet and Computer User Agreement”.
2. Eating, vandalism, smoking and drinking are prohibited in the computer laboratory, CISCO, troubleshooting
and Hardware rooms.
3. Only enrolled students are allowed the use of the Computer Laboratory.

4. Students without laboratory schedule may be allowed to use the computers depending on the availability of
computer units as approved by the laboratory supervisor or laboratory staff. Students must request permit from
the laboratory personnel and pay corresponding fee(for non- CSD programs).
5. Use of laboratory computers is for class related work. Please limit recreational use of the computers i.e.
Internet, chat rooms, e-mail and social-networking sites. Use of internet for any commercial purpose, and for
the purchase or sales of items is also prohibited.
6. Only one computer at a time may be used. Personal computers, laptops must not be connected to the lab.
7. Playing computer games, other than ones intended for teaching, are not allowed.
8. Computers are configured according to the needs of students and faculty/staff. Users must not change
hardware or software configuration settings/preferences including but not limited to changing of wallpaper,
system password, BIOS password and the like. Users who have suggestions for a hardware or software
configuration change should speak to the Laboratory Supervisor.
9. Report immediately any malfunctioning equipment and any suspected problems with software and/or
equipment to your professor so that your professor would make necessary endorsement to the laboratory
personnel. In case of the absence of professor, you may report it directly to the laboratory personnel. Do not
attempt to repair or disassemble any laboratory equipment. Failure to report the problem, the student assigned
to the unit will be held responsible for the consequences.
10. The computer users shall assume full responsibility for his action on the computers and or network; and agrees
to take proper care of all equipments, hardware peripherals, manuals, kits, accessories and other property of
the COMPUTER STUDIES LABORATORY and to pay replacement cost of any equipment, hardware
peripherals or materials lost or damaged as a result of the carelessness, malicious intent, or failure to inform
faculty member/Laboratory staff of malfunctions.
11. Obstructing other people's work by consuming unreasonably large amounts of system resources(any attempt to
cause degradation of system performance, unduly increase network load), or by deliberately crashing any
CSD- Computer Laboratory computer system, damaging of hardware and computer equipment and materials
will have corresponding sanctions.
12. Do not utilize computing resources for illegal, malicious, destructive purposes, intentionally or unintentionally
damage or disrupt any computing resource process.
13. The use of any workstation for any illegal or criminal purpose is prohibited. This include but not limited to the
following:
a. Intentionally or recklessly introducing or transmitting destructive or malicious programs such as
viruses, “worm”, “Trojan horse”, and the like into the computer system, network or networked devices.
b. Revealing account passwords to others or allowing use of accounts by others.
c. Attempting to obtain or use, passwords, IP addresses or other network codes that have not been
assigned to you. Further, attempting to obtain unauthorized access to computer accounts, software,
files, or any other University IT resources.
d. Using any program/script/command, or sending messages of any kind, with the intent to interfere with,
or disable, a user's terminal session, via any means, locally or via the network.
14. The following are rules for printing:
a. Users may print 5 pages per subject per semester, free of charge. For additional print outs, they are
required to bring their own paper and pay for each pages printed. Printers shall not be used to make
multiple copies of a single document. Payment in advance shall be made. See Payment procedure.
b. Users should be responsible to preview the document and determine the number of pages. No refunds
will be given for blank, unwanted, or improperly formatted pages.
c. A page is defined as one side of a sheet of paper. Although users have the option of duplexing (printing
on both sides of a sheet), they will still be charged 5pesos per side.
d. Users may supply their own paper if it falls within the printer specifications, but they will still be
charged 5 pesos per page for printing.
e. A maximum of 30 copies per day may be made using the LABORATORY printers, regardless of
whose paper is used. This limit will be strictly enforced, and violators will be barred from further use
of the CSD- Computer Laboratory 's printers.
f. CSD- Computer Laboratory staff/Faculty may need to suspend a print-out that extends beyond the
user's computer time or the CSD- Computer Laboratory 's public hours.
15. Download files shall be saved to your designated removable disks.
16. Only authorized staffs are allowed to install any software.
17. All software is protected by the Copyright Laws and/or Patents and/or Trade Secrets Licenses. Users should
not copy/duplicate any software, computer programs or documents without authorization. Hence, doing such
may result in civil and criminal penalties, including fines and imprisonment.
18. When you leave, please log off your computer. Computer left unattended for more that 5 minutes may be
logged off.
19. Please show respect and consideration to faculty and staff, including interns and laboratory assistants.
20. Persons who are disruptive in computer laboratory facilities will be asked to leave immediately. Repeated
incidents of disruptive or harassing behavior will result in loss of computer use privileges.
21. Laboratory assistants are not responsible for personal items left in the computer laboratory rooms. Items that
are found will be forwarded to the SWAFO.
22. All decisions as to the use of the computer hardware and software are at the sole discretion of the laboratory
supervisor. In all cases, including exceptions to the above rules, the decision of the supervisor will be final.
Failure to abide by the decision will result in the possibility of legal liability being incurred by the computer
user.
23. Computer laboratory staff members will provide assistance in the operation of the various software packages
installed on the laboratory computers. However, laboratory staff members will not solve homework problems
for students, nor can they spend so much time assisting individual users that they neglect their other laboratory
works.
24. Procedure on Payments:
a. Secure payment slip at the Hardware Laboratory.
b. Fill-up the form and pay the corresponding amount and secure O.R at the University cashier’s office.
c. Show the O.R to the hardware laboratory personnel.
25. Borrowing and Returning of Computer laboratory equipments/materials/tools
a. *Students submit their laboratory ID to the laboratory tech for borrowing purposes. The student
should check carefully the borrowed laboratory equipment/materials/tools before receipt. Any defect
should be reported to the laboratory before leaving the counter, otherwise, any defect found after
leaving the counter will be charged against the borrower.
b. The borrowed laboratory equipment/materials/tools should be returned in good working condition to
the hardware laboratory by the student at least 30 minutes before the end of laboratory period.
c. In case of lose/breakage/damage of borrowed equipment the student concerned should inform the
laboratory technician immediately and prepare an incident report within 72 hours.
d. In case of laboratory deficiencies, the laboratory ID of the student concerned is withheld in the
hardware laboratory until such time that the account is settled.
e. Any faculty/student who wishes to borrow laboratory equipments/materials beyond the laboratory
schedule for valid purposes must strictly follow the rules on reservation of equipment/materials.
26. Actual or suspected violations of this policy or related rule should be reported to the Hardware Laboratory. In
reporting a violation, complainants should cite the specific section of this policy that has been violated. Failure
to resolve the situation will result on the filling of formal charges thru the appropriate process.
The user acknowledges that he/she has read and understood these rules and promise that will abide by them. Failure to
observe any part of the "Internet and Computer User Agreement" may result in a suspension of the user's right to use
the computer, and/or disciplinary action including but not limited to reprimand suspension, probation, expulsion,
termination and legal or civil action.
