2. If documentation necessary for membership is found not to be in a members file at the time of a request for an
update ID card that documentation must be furnished before a new ID card will be issued.
3. If you are requesting an updated or replacement ID card you must attach a colour passport photograph with a
white background taken within the last 90 days to this form. Home and black and white photos are not accepted.
4. Passport photos are available at various photo shops and Walmart.
5. Height and weight must be in Centimeters and Kilograms.

remains the property of the Ardoch Algonquin First Nation and must be surrendered upon request of the Family
