In case of illness or injury, I, the undersigned, do hereby give my consent for the doctors and staff of Roper Mountain Animal Hospital to treat, prescribe medications, or perform surgery on my pet(s) while they are boarding. All reasonable efforts will be made to contact the owner at the emergency phone number listed before proceeding. Roper Mountain Animal Hospital is to use all reasonable precautions against illness, injury of escape of my pet(s). Should my pet(s) remain unclaimed after the agreed upon discharge date, I agree that my pet(s) may considered abandoned. Written notice of abandonment will be mailed to my home address. If my pet(s) remain unclaimed, I agree that I release all claim to my pet(s) and that it may disposed of as the hospital deems necessary. Abandonment will not release me from payment if services rendered. All pets must be current on vaccinations. Dogs: DHPP, BORDETELLA, RABIES Cats: FVRCP, RABIES. Pets without proof of vaccination administered by a licensed Veterinarian will be vaccinated at the owner’s expense. All pets must be free of external parasites (fleas, ticks, ear mites, etc.) Pets with external parasites will be treated at the owner’s expense. Puppies and kittens under 16 weeks of age are more susceptible to diseases, infections and parasites than older dogs or cats. While every effort will be taken to avid exposure, these pets are at higher risk wile boarding I understand and assume all risks associated with boarding a puppy or kitten. Pets will be checkedin and discharged for boarding only during normal business hours. The is for the safety and well-being of your pet, as fleas and ticks can be outside when we are walking them.
