Khiron House Outcome Measure Analysis Independent Data report prepared by Elspeth Twigg, CORE IMS Research Lead February 2014 The Sample Because the assessment date field was not routinely completed, clients were filtered by post session date. The sample consisted of 19 clients whose post session date lay between 1/10/12 and 31/12/13. 13 clients were inpatients and 6 outpatients. [Khiron House note: outpatients participated in this study voluntarily and so only a small sample was collected. This will be improved for 2014] Pre-therapy Assessment outcomes Accepted for therapy No data Total Inpatient 11 (36%) 2 (64%) 13 Outpatient 3 (25%) 3 (72%) 6 All 14 (74%) 5 (26%) 19 Since all of the clients in the sample had Post session dates, all must have been assessed and thus should have had an assessment outcome. Gender Inpatient 3 (23%) 10 (77%) 13 Male Female Total Outpatient 1 (17%) 5 (83%) 6 All 4 (21%) 15 (79%) 19 The ratio of female to male clients was 79:21, with the proportion being slightly higher for outpatients (83%) than for inpatients (77%). Proportion of clients above/below cut-off pre-therapy Above cut-off Below cut-off Total Inpatient 12 (92%) 1 (8%) 13 Outpatient 6 (100%) 0 6 All 18 (95%) 1 (5%) 19 Of the 13 inpatients, 12 (92%) were above the clinical cut-off pre-therapy and 1 (8%) was below. All of the 6 outpatients had scores above the clinical cut-off before therapy. Pre-post analysis Pre-Post outcome measure completion All clients in the sample had pre- and post-therapy OM data. However, this is likely to be due in part to the fact that clients were filtered by Post session date. It is highly likely that other clients were also seen in the period but not included in the sample because the relevant data fields had not been completed. [Khiron House note: All clients who had completed at least 2 outcome measures and were no longer in treatment at 31/12/13 are included in this study.] Pre-Post outcome measure score change 17 clients had pre-post CORE-OM data. Inpatients (n=12) Pre-therapy Post-therapy Pre-post change Range 8.5 – 30.0 9.1 – 24.7 -0.6 – 11.8 Mean (SD) 21.3 (6.12) 15.1 (5.79) 6.2 (4.16) The average change in score for the inpatients in the sample who had CORE-OM data pre- and posttherapy was a reduction of 6 points. Outpatients (n=5) Pre-therapy Post-therapy Pre-post change Range 15.5 – 27.7 1.8 – 24.7 -4.7 – 13.5 Mean (SD) 21.8 (5.05) 14.7 (9.69) 7.1 (7.59) Outpatient clients with pre-post CORE-OM data showed an average reduction of 7 points. Outcome Measure analysis Reliable improvement A reliable change in score is a statistically significant change in score between the start and end of treatment. That is, the change in score is not likely to be due to random fluctuations in score over time. The threshold levels for reliable change are 5 or more points in CORE-OM clinical score, or 6 or more points in CORE-10 clinical score. Once again, analysis was only conducted on those clients with pre- and post-therapy outcome measure data. Reliable change Reliable deterioration No reliable change Reliable improvement Total Inpatient 0 5 (38%) 8 (62%) 13 Outpatient 0 2 (33%) 4 (66%) 6 All 0 7 (37%) 12 (63%) 19 We can see from the reliable change rates that a substantial proportion of inpatients (62%) showed reliable improvements in score between the start and end of treatment. A similar proportion of outpatient clients (66%) showed reliable improvements in score between starting and finishing therapy. Clinical recovery Clinical recovery is a move from above to below the clinical cut-off (10 for the CORE-OM, 11 for the CORE-10) between the start and end of end of therapy, and thus a move from a clinical to a nonclinical population. Clients who showed no clinical recovery are those whose scores remained above cut-off at the end of therapy. Analysis was only conducted on those clients who have pre- and post-therapy data. Clinical change Below cut-off (pre) No clinical change Clinical recovery Total Inpatient 1 (8%) 9 (69%) 3 (23%) 13 Outpatient 0 3 (50%) 3 (50%) 6 All 1 (5%) 12 (63%) 6 (32%) 19 Only 23% of inpatients showed a clinical change from clinical to non-clinical in their outcome measure scores between the start and end of treatment. This figure was rather higher for outpatients at 50%. This may in part be due to the high proportion of clients whose scores were above clinical cut-off pre-therapy. [Khiron House note: Full clinical recovery is not routinely expected during inpatient treatment. The goal of inpatient treatment is to improve the client’s condition ready for outpatient treatment.] individual client CORE-OM outcomes - Khiron House inpatients 35.00 30.00 25.00 20.00 15.00 10.00 5.00 0.00 Pre-therapy OM clinical score Post-therapy OM clinical score individual client CORE-OM outcomes - Khiron House outpatients 40.00 35.00 30.00 25.00 20.00 15.00 10.00 5.00 0.00 Pre-therapy OM clinical score Post-therapy OM clinical score green = reliable and clinically significant improvement; blue = reliable improvement; orange = clinical improvement; grey = no reliable change
