Laker Tennis Player Apparel Form

If interested in playing tennis this spring, we are asking each player to pay a flat
rate of $40.00 to cover the costs of apparel. This includes a T-shirt, uniform top,
and sweatshirt. The T-shirts and sweatshirts will be a Graphite Gray with red and
white design; a great way to show your support. Our uniforms will be red, but
with the same design. Please fill out the form below and include payment. All
proceeds will go toward the Calloway County Tennis Team.

Please attach cash or check to this order slip and return to Maria Vincent
(Guidance Office) or Coach Price no later than Thursday, February 23rd. Make
checks payable to Calloway County Tennis.
If family wants to buy apparel, we will have a separate order form with prices
listed for each.
