SECTION 1: PURPOSE OF THIS JOB:
As a member of the Specialist Methamphetamine Team for WCADS, the purpose of this position is
to:


Participate with team members in the delivery of treatment programs utilising nursing skills
Provide clinical assessment and intensive case management support to clients via the
implementation of a recovery plan that involves mobilising the resources of other community
agencies

Clinical Case Management
Key Responsibilities


Apply advanced clinical knowledge and skills in clinical assessments, program delivery and
referral for clients who are experiencing problems with methamphetamine and other drug
dependencies, using a holistic (whole of life) approach.

Demonstrate effective skill and knowledge in the delivery of nursing care and treatment in
the area of methamphetamine presentations as well as other drug and alcohol presentations.

Plan and provide intensive clinical case management for individuals, groups, couples and
families affected by methamphetamine and other drug dependencies.

Participate with team members in the delivery of treatment programs utilising nursing skills.

Identify, plan, implement and evaluate appropriate clinical treatment programs in conjunction
with other members of the team and other agencies.

Undertake a triage function as required for clients.

Provide comprehensive assertive follow-up for clients on caseload.

Ensure appropriate risk management strategies are employed, including identification,
assessment, investigation, and control and monitoring.

Make professional, clinical decisions in consultation with the specialist team and counsellors.

Ensure that recovery plans, case notes, clinical reviews and client data are maintained to the
required standards.

Provide professional guidance and support to counsellors as required, including on
complex/difficult cases.

Provide services in a range of locations that may include on-site and community locations.

Function in accordance with all aspects of Legislation affecting Nursing practice.

Ensure that clinical services delivered reflect good practice standards and are in line with
Holyoake Philosophy, Policy and Procedures.
Community and Stakeholder Engagement
Key Responsibilities

Ensure an integrated care approach by promoting effective working relationships with other
services, health professionals and community agencies.

Identify key stakeholders for engagement to support the objectives of the Specialist
Methamphetamine Project.

Engage in the development of collaborative partnerships with stakeholder agencies as
identified.

Assist in the development of formal Service Level Agreements and/or MoU’s with key
stakeholder agencies as identified.

Contribute to the development of clinical policies and procedures.

Attend meetings with stakeholders to facilitate effective working relationships.

Contribute to the preparation of educational and/or promotional material for community
distribution as required.

Assist the Senior Clinical Case Coordinator to ensure the effective operation of case
management and case review processes within the team.

Participate and support the evaluation of the Specialist Methamphetamine Project, as
undertaken by NDRI.

Inform the line manager of any issues arising in relation to service delivery, including
feedback from consumers.

Effectively manage appointments with clients and community based agencies.

SECTION 4: AUTHORITY LEVELS:
The Clinical Nurse operates under the general direction of the Regional Manager. This position
has a significant degree of autonomy and responsibility in terms of complex case management
and client support.

SECTION 5: COMPETENCY REQUIREMENTS:
SELECTION CRITERIA
Qualifications and Training:
 Eligible for registration in the category of Registered Nurse by the Nursing and Midwifery
Board of Australia with significant experience as a Registered Nurse.
Knowledge and Experience:

Demonstrated understanding of alcohol and other drug (AOD) issues including comorbidity
(co-occurrence of AOD and mental health issues).
Skills:

Demonstrated assessment and complex case management skills including effective time
management and personal organisational skills.

Ability to deliver services in a culturally appropriate and secure manner.

Ability to work effectively as a team member, and to support and mentor staff.

Good interpersonal, communication skills and demonstrated problem solving skills.

Ability to work within the Holyoake values and philosophy.

Demonstrated proficiency with Microsoft Office software package including Word and
Outlook.
Other:

Evidence of current registration by the Nursing and Midwifery Board of Australia.

Current National Police clearance.

A current Working with Children Check if required.

Other security clearances if required.

Current WA Drivers licence.
