THIS RELEASE IS A CONTRACT WITH LEGAL CONSEQUENCES. READ IT CAREFULLY BEFORE SIGNING. ALL
ARTS AND CRAFTS PARTICIPANTS MUST COMPLETE THIS FORM AND SUBMIT TO NAU.

Acknowledge and fully understand that I will be participating in activities that may or may not involve risk of serious injury,
permanent disability, property damage and/or death. These risks may result not only from my own actions, inactions, or
negligence, but also from the actions, inactions, or negligence of others. Further, there may be other risks not known to me, or
not reasonably foreseeable, such as disability or death.
Assume all the foregoing risks and accept personal responsibility for any damages following any such injury, permanent
disability, property damage, or death.
Release, waive, discharge, and covenant not to sue the State of Arizona, the Arizona Board of Regents, Northern Arizona
University, their officers, employees, and agents, and their heirs, administrators, and executors, from demands, losses or
damages on account of injury, including death, or damage to property, caused or alleged to be caused in whole or in part by the
negligence of any person or otherwise, for myself and my spouse, if any, and our heirs, successors, and assigns.
Understand that the State of Arizona, the Arizona Board of Regents, and Northern Arizona University will not be responsible for
damage or theft of property owned by me or in my care and custody.
Understand that the State of Arizona, the Arizona Board of Regents, and Northern Arizona University do not provide medical
coverage to a participant if injured while participating in the event described above or attendant activities. Any medical costs
incurred as a result of this activity will be my financial responsibility.
(The following is applicable to vendors selling pre-packaged food products at their booth) If I sell any pre-packaged food
products at my booth, I fully understand that I must first notify the NAU Arts and Craft Fair committee of my intentions to sell
pre-packaged food products. I will provide a list of all food products to be sold, and I am aware I must receive approval from
NAU to sell food products prior to date of the event. I further understand that I must adhere to all county, state, and federal
rules and regulations regarding health standards for the general public. I assume all responsibility for such compliance and
understand that permission to do so is limited to this single event. I understand that I must obtain a Temporary Food Service
License or applicable license/permit from Coconino County Health Department for the preparation and sale of my food
product(s), and for providing free samples of my food product(s). The cost of the permit is my responsibility. I will provide
NAU with a copy of the License and a list of food products to be sold when I send in my booth payment. I understand I may be
required to provide NAU with an approved Certificate of Insurance that meet specific state insurance requirements and limits
prior to the event.
ACKNOWLEDGE THAT I HAVE READ THE VENDOR POLICIES, UNDERSTAND ITS CONTENTS, AND AGREE
TO ABIDE BY THESE POLICIES.
ACKNOWLEDGE THAT I HAVE READ THE ABOVE RELEASE FORM, UNDERSTAND THAT I HAVE GIVEN UP
SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT VOLUNTARILY.
