Your Beneϐits…Your Choice Each fall, we ask you to make beneĮt elecƟons for you and your eligible dependents. As you get ready for the 201ϲ Open Enrollment period, here are three important steps you should take to ensure you have the coverage you need on January 1, 201ϲ. Review your beneĮts—and learn what’s new. As you can see from the chart on page 2, there are changes. This open enrollment guide provides details on these changes with addiƟonal informaƟon about all of the beneĮts available to you. The 201ϲ Jarden Employee BeneĮts Guide provides more detailed informaƟon. Please take the Ɵme to review your opƟons and the changes so you will be prepared to make informed decisions. Consider your needs for 201ϲ. The Ɵme you take to “do the math” may result in hundreds if not thousands of dollars of savings and beneĮts. Start by asking yourself these quesƟons: Do I have access to other group medical insurance? If yes and you enroll in that coverage, Jarden will provide a $50 per month Opt-Out Credit that you can apply toward the cost of other beneĮts opƟons. Do I have enough health, survivor beneĮt, and income protecƟon insurance to protect my family in the event of the unexpected? Not sure? Then, use the tools and resources available to you to compare your opƟons and determine which coverage is right for you. Make informed decision. Once you’ve reviewed your 201ϲ beneĮt opƟons, gathered up your dependents’ Social Security numbers, dates of birth, and addresses and go to beneĮtsŚĞůƉ.jarden.com. During your open enrollment period, you can enroll anyƟme͕ seven days a week. See the sidebar on the top leŌ of page 2 for your enrollment dates. Follow the instrucƟons on page 15 to enroll in beneĮts and update your beneĮciaries. Action Required During Open Enrollment We recommend that you enroll in all of your beneĮts during this open enrollment period to make sure you have the right coverage. If you do nothing during open enrollment, most of your 201ϱ beneĮt elecƟons will carry over to 201ϲ, except your spending account elecƟons (HSA and FSAs). This includes union and non-union employees. (Please note, medical and dental contribuƟons for part-Ɵme employees are higher.) You can enroll your eligible dependents in any plan that oīers dependent coverage provided you enroll yourself as well. Your eligible dependents include: x Your spouse. However, if your spouse is employed and is eligible for coverage under his or her employer’s group medical plan, he or she is not eligible for coverage under a Jarden medical plan. x Each dependent child, up to his or her 26th birthday (may vary by plan), unless the child is incapable of self-sustaining employment by reason of a mental or physical handicap and is chieŇy dependent on you for support and maintenance.
