Zampell requires proof of the following insurance for all contractors working for us:
1. General Liability on an “occurrence” form covering all locations and operations of the contractor on
ISO CG0001 or equivalent. That form includes coverage for ongoing operations, products /
completed operations, independent contractors and contractual liability as well as other coverages
such as personal injury and advertising injury. We will not accept any restrictions on contractual
liability coverage.
2. Automobile liability with limits of at least $1 million CSL covering “all autos” (symbol #1) or owned,
non owned and hired (symbols 7, 8 & 9).
3. Workers Compensation and Employers Liability providing statutory coverage for WC and with limits
of at least $500k/500k/500k for employers’ liability. This policy must cover the state where the
contractor is headquartered and the state where work is being performed (MA). Your agent should
show on the certificate the states where coverage applies.
If the contractor’s primary limits on any of these policies are lower than the required limits above the
deficiency may be supplied through an umbrella or excess liability policy. When listing the Umbrella / Excess
Liability policy on the certificate of insurance (COI) it must state that it is excess of each of the policies above.
Zampell requires that the following entities are included as Additional Insureds on your GL and Auto Liability
policies:

Contractors on this job will be working for J.C. Zampell Construction, Inc. which is the GC for the job, but are
also required to show the other upstream entities as Additional Insureds. Your GL policy will therefore need
to include Additional Insured endorsements equivalent to ISO CG2038 and CG7921 (for “ongoing operations”
and “completed operations” respectively) (CG2010 and CG2037 are also acceptable but they are “scheduled”
forms and we require either a copy of the COI or that the COI specifically show the wording used to complete
the schedules on the endorsements).
Zampell requires the Certificate Holder to be:
