RISK ASSESSMENT FORM Activity: Laserzone/quest Form completed by: Natasha McInninie Location: Any Nov 2014 NOTE: The following information is specific to the activity above and Mentors should be aware of the guidance given in the General Mentoring Risk Assessment which should be used in conjunction with this form. Mentor to be aware of the rules, regulations and restrictions of any Laserzone/quest facility prior to visit. Hazard Injury Nature of Risk. Who might be harmed? Mentor to ensure young person understands all rules and regulations associated with the game and listens to briefing given by Games Marshall at start of the game. Befriender to check suitable clothing is worn (ie no loose clothing and suitable footwear worn). Mentor to be aware if the game features strobe lighting. As indicated on general risk assessment mentor to ensure that they carry ID card on each outing and to remain with young person to respond quickly to any injury Mentor to ensure young person listens to Games Marshall at start of game and is aware of all rules and regulations. Mentor to ensure young person is aware that no running, climbing or physical contact is allowed during the game.
