First Serve OKC Tennis Scholarship Requirements:
General Information:








Eligibility is based on: financial need; commitment to excellence in the classroom, at First Serve
OKC, and in the community; and outstanding character.
Applicant must be enrolled for 2016-2017 in a grade of 1st through 12th.
Applicant must attend the public school system.
Applicant must have completed the First Serve OKC program application.
Applicant must have a 2.5 grade point average where applicable, or satisfactory grades if in
elementary school.
Applicant must demonstrate positive behavior and tennis etiquette at all times.
Applicants’ parent(s)/guardian(s) tax return or income verification documents will be reviewed,
and scholarship funds will be awarded based on a sliding scale. See attachment for details.
Scholarships may be revoked if, in the sole discretion of the First Serve OKC Scholarship
Committee, the recipient fails to meet and uphold these requirements.
Demonstration of Financial Need
Submit copy of the family/guardian’s most recent tax return (1st page of 1040), OR another form of
income verification (most recent two pay stubs). If neither are available, attach a description of the
family/guardian’s financial circumstances that demonstrates a financial need for scholarship.
Demonstration of Excellence in the Community
Attach answers to the following questions in 250 words or less (preferably in student’s words;
parents/guardians may help younger students):
1. What goals do you want to accomplish in tennis as well as in the classroom?
2. How will you use what you learn at First Serve OKC to help others?
Demonstration of Academic Excellence
Include the applicant’s most recent report card. Attach additional pages describing any special
circumstances if the report card does not sufficiently demonstrate the applicant’s aptitude and/or
commitment to academic excellence.
