 Makes payment either by E-Check or Credit/debit Card
 On successful submission following PDFs will be generated:
 Payment Receipt (If fee is not $0)
 Request Form (Inspection Request Form and Amendment Request Form)
 Email notification will go to the applicant and the Owner.
NOTE: Applicant can see the generated PDFs on clicking the PDF icon on
My Request page.

On Final Decision
 On Approval:
 An Approval Notice will be generated in a PDF format as per the
request (Certificate To Operate and Amendment Request Approval Notice)
 Email Notification will go to the applicant as well as to the Owner for all
types of requests (Inspection Request, Amendment request and Affidavit
request)
 On Disapproval / Objection:
 Notice Of Installation Disapproval will be generated in a PDF format.
(Only for Inspection Requests)
 Amendment Request Disapproval Notice will be generated in a PDF
format. (Only for Amendments)
 Email notifications will go to the applicant as well as to the Owner for all
types of requests (Inspection Request, Amendment request and Affidavit
request)

Basic Instructions
 If applicant selects ‘Tax Exemption’ or ‘Fee Exemption’ feature, attachment
of proof is mandatory (only for Tax Exempt).
 Performance Test Worksheet (PTW) along with combustion teat data
printouts is mandatory for Inspection requests.
 All attachments should be in a PDF format.
 All required attachments should be done before clicking the ‘Submit’ icon .
 If an Amendment is filed for editing the Owner Name, attachment of
Ownership is mandatory.
