Position Summary: The Team Leader – Katherine is responsible for: • The coordination of all Commonwealth Home Support Program services delivered in Katherine. This includes Centre based Respite and Flexible Respite. • The coordination of supports under NDIS. • Coordination of staff matched to deliver services. • Under the guidance of the Manager - Aged Care Services ensure that all staff deliver services within a consumer directed care framework with a focus on outcomes for the client • Conducting assessment, care planning, and review of clients eligible for services. • Ensuring all activities and planning relating to clients individual identified goals and skills contained in their care plan occurs. • Ensuring delivery of services to the client are undertaken in a way that respects the dignity, privacy, independence and cultural aspects of the client. • Lead by example by modelling the values, attitudes and behaviour associated with the position at all times. Position Responsibilities The Team Leader is responsible for: 1. Supporting the Manager - Aged Care Services with the progression of Aged Care Services strategic work plan 2. Identifying quality improvements/recommendations and the smooth operation of Aged Care Services by reviewing operational procedures, recommending changes and assisting with routine administrative tasks. 3. Ensure services are delivered in line with the Commonwealth Home Support Program Manual 4. Utilising a wellness approach ensure the delivery of services in a consumer direction model of service delivery that focuses on a client’s life goals and strengths and supports the maintenance or restoration of the client’s independence. 5. Assess clients eligible for services and ensure information is provided to match support worker’s skills and interests for the progression of client’s goals and outcomes. 6. Identifying rostered hrs required and support needs of clients to arrange support workers for delivery matched to client needs. 7. Ensuring the successful every day running of the centre based respite including shopping, activity planning, collection of fees, record keeping etc and communication to relevant staff 8. Ensure the successful running of flexible respite including camps and remote troopies. 9. Completing client reviews and evaluation of client’s progress toward expected outcomes 10. Coordinate of the Commonwealth Home Support Program to ensure successful outcomes, including meeting expected outputs of the program and collation documentation to support reporting requirements. Position Selection Criteria: Essential: • Relevant tertiary qualifications in Aged Care or similar and/or demonstrated relevant experience (Minimum 2 years) . • Sound understanding of consumer directed care • Understanding of wellness and re-enablement principles and how they apply to the delivery of aged care services • Good understanding of aged care service provision • Experience working with remote communities Demonstrated: • High level interpersonal and communication skills. • High level assessment skills • Ability and motivation to take a leadership role • Ability to organise and plan personal workload be creative and show initiative • High level computer skills (Word processing, database, Internet, email) • Willingness to undertake duties in a way that contributes to a positive impact on the organisational culture.
