Shaw AFB Mishap
• In Dec 2009, a sub-contractor employee was crushed between a
second-story steel I-beam and a falling steel roof I-beam.
• Other workers were attaching a second beam to the other side
of the column supporting the beam that fell, no “seat” was in place
under the first beam.
o Prime ktr had a subcontractor employee killed on a separate DoD contract
the previous year.
o The prior subcontractor death did not reflect in prime’s EMR.
o 45 days of lost productivity due to mishap investigation.

MCLB Albany Mishap
• In Mar 2011, a laborer in Albany was shocked when he
touched the headache ball on a crane which had contacted an
overhead power line.
• Victim’s heart stopped three times, but he was revived each time
and is recovering.
o The victim was an employee of a second-tier sub-contractor that had an
EMR of 1.33
o Would not meet current sub-contractor safety criteria.
o Two months of lost productivity due to mishap investigation.
