Learning Disability and Vulnerable Adults Service
The service is committed to enhancing the quality of life for people with a learning
disability and also vulnerable adults who may have or have been assessed to have
eligible social care needs.
The team's philosophy is to support service users to have control over their daily
lives by using Personal Budgets as much as possible which includes supporting
people to live independently in their own home (where appropriate).
The service supports to some of the borough’s most vulnerable citizens.
Personalisation is at the heart of how each team operates.
Who can make a referral to the service?
Anybody can make a referral to the learning disability and vulnerable adults’ service
Young Adults Team
The Young Adults team is an integrated health and social care service which
supports young adults in their preparation for adulthood from the age of 14 in order
to make sure the move to Adult Services is as smooth as possible.
The team works in collaboration with Children’s Social Care, education and health to
ensure young people have an education, health and care and support plan.
The team also undertakes preparing for adulthood assessments where it would be of
significant benefit to those aged 14 – 18 years old.
Ongoing Support Team
This team provides an assessment and review service to adults with learning
disabilities and vulnerable adults with eligible care and support needs
The team has a focus on helping people to identify their wellbeing outcomes and
how these can be achieved through the use of a personal budget.
Complex Cases Team
The complex cases team provides an assessment and support plan service for
adults who have high dependency needs and / or may have behaviours that
challenge.
The team also carries out specialist assessments and links closely with the NHS
Continuing Healthcare team (CHC)
Learning Disability Partnership Board
In Rochdale, we have a Learning Disability Partnership Board. This Board makes
sure that we have the right support in the borough to help people achieve the life
they want.
