CONSENT TO USE OF PHOTOGRAPHS/IMAGES OF JUNIOR PLAYERS
I consent/do not consent (cross out that which does not apply ) to the use of photographs/images of
the player named below (Player Name) taken during games or training sessions for display on the
HTCC website, Official Facebook Page or in the club newsletter. In the event that I wish to withdraw
my consent, I undertake to inform the club of this in writing.
I understand that in the instance that the HTCC wishes to attach identifying information about
individual players to such images, separate consent will be sought from the player’s parent or
guardian.
