What does our service offer?
Neurological rehabilitation
Community Stroke Services:
We are a specialist, community
rehabilitation
and
supported
discharge service currently based at
The Cedars Unit, Tolworth Hospital.
Our service offers comprehensive,
specialist
assessments,
treatment,
advice & education on issues relating
to neurological conditions including:
Our service provides multidisciplinary,
neurological
rehabilitation
and
disability management to adults with
acquired and long-term neurological
conditions, helping them to maximise
both independence and quality of
life.

This is a community based rehabilitation
service that enables acute, stroke
patients discharged from hospital stroke
units, and meeting our eligibility criteria,
to have intensive rehabilitation at home
for up to six weeks to help maximise their
function as quickly as possible.
The CNRT core team comprises of:

Speech and Language therapy

Occupational therapy

Physiotherapy

Psychology *

Dietetics *

Therapy Assistants

Links to specialist services
Our team works with patients and
carers in a variety of settings where it is
most clinically effective. This can
include the patient’s own home, as an
outpatient at Tolworth Hospital, or in
any community setting such as gyms,
day centres, place of work etc.

We offer a goal-orientated approach
and individualised care packages to
meet each patient’s needs.
We are a time limited service and our
intervention varies from 6 to 12 weeks
and intensity can be 1-3 sessions per
week depending on need.
Management of Long Term Conditions
This service provides for people with
progressive
and
non-progressive
neurological impairments to facilitate
self-management and preventative
support. Treatment intensity can vary
from once a week to once a month.
Early Supported Discharge (Stroke)
Acute, stroke survivors not meeting the
ESD criteria can also receive stroke
specialist rehabilitation at a more
suitable intensity which still allows the
individual to achieve their goals and
maximise independence.
In patient Stroke Beds
Stroke survivors who still require a high
level of support and are unable to be
discharged home from the acute stroke
unit, but have continuing rehabilitation
goals can be admitted to these beds.
They will receive intensive stroke
specialist therapy for a period of on
average six weeks.
