5/12 Scholarship Information
PURPOSE:
To provide 5th grade at-risk students an on-going financial incentive to attend postsecondary education through a cooperative effort by the Greater Green Bay Chamber
and the Greater Green Bay Community Foundation.
ACTION PLAN:
This scholarship qualification process will operate on a continuous basis beginning with
Grade 5 and following the student through successful completion of Grade 12.
 $200 will be deposited in the student’s scholarship fund at the end of the 5th
grade; $100 will be deposited each successfully completed year from 6th – 11th
grades; $200 will be deposited at the end of 12th grade for a total scholarship
amount of $1,000.
 If a student has difficulty one year (per the below listed eligibility criteria), the
student will have an “honor” year, which means no money will be deposited in
the scholarship account. If this student has difficulty a second year, the student
will be removed from the program and the scholarship money will go back into
the general fund.
 A student moving out of Brown County may remain in the program if yearly
progress reports from the student’s current school district are submitted to the
Greater Green Bay Chamber. If no report is submitted for the first year after
moving out of Brown County, the student will be removed from the program. If
yearly progress reports do not continue to be submitted, then removal from the
program will also occur.
 Their post-secondary institutions will receive one-half of the scholarship for the
first semester and one-half for the second-semester of post-secondary education.
Checks will be made out to the post-secondary institution on behalf of the
student.
RESPONSIBILITIES:
 Partners in Education of the Greater Green Bay Chamber will be responsible for
selection procedures and monitoring processes relevant to the total process.

 Sponsors for the program will be community organizations and businesses.
 Greater Green Bay Community Foundation will execute the financial
responsibilities.
 Individual school districts will be directly responsible for student selection and
annual evaluation.
SELECTION and SCHOLARSHIP ELIGIBILITY CRITERIA:
Nominate at-risk students who demonstrate strong potential, academic
achievement, personal involvement, excellent school attendance and financial
need. Participating students will be selected by Partners In Education representatives
and notified before the end of the current school year
PARTICIPATING SCHOOL DISTRICTS:
Ashwaubenon, De Pere, Denmark, Green Bay, Howard-Suamico, Luxemburg-Casco,
Pulaski, Seymour, West De Pere, Wrightstown and GRACE schools
