Congratulations and welcome to the Rock Paper Scissors Birthday Club! It is simple to join and you
will be so happy you did! Just fill out the form at the bottom of this page and you become an
official member. We take our birthday club very seriously, because there is no kidding around when
it comes to celebrating a birthday.
As a birthday club member you will:
 Be mailed a special birthday gift certificate worth $10 prior to your birthday.
 Receive additional $5 gift certificates for every gift purchased off your completed wish list
(attached). In order to receive the $5 you must fill out a wish list 2 weeks prior to your B-Day.
 Be eligible to have your very own Birthday Wish List page on our website.
 Be eligible to play "Rock Paper Scissors" vs. a store employee and pick a prize out of our
treasure chest.
Please note:
• The Birthday Club is for any family member turning 2 or older.
• Gift certificates can only be redeemed one at a time.
• All information will be held in strict confidence and will not be sold or given to any third party.
• We send all Birthday Cards & Gift Certificates by the United States Postal System.
• B-Day cards are to be redeemed by the B-Day person only.
• Rock Paper Scissors is not responsible for any lost gift certificates or address changes.
