PVI vs LEB for infrageniculate disease in
patients with CLI
Hicks et al, Vasc Med 2017;22:28
Johns Hopkins
VQI Data
2566 patients;
500 LEB, 2066 PVI
Outcome measures @ 1 year:
Primary patency
Major amputation
Mortality
PVI vs LEB for infrageniculate disease
in patients with CLI
No difference in:
Major amputation
Mortality
Superior primary patency: PVI
The details…
38% of bypasses to DP or plantar vessel
0% of PVI procedures involved DP / plantar
Median # of vessels treated in PVI group: 1
PTA alone in 85%
”Majority” of bypasses with autogenous vein
? SSGSV
? Spliced vein
? Cephalic vein
Very likely that many of these LEB patients had no
endo solution

Total Variable Cost
Why is this important ?
ACO era (capitated payment)
System’s bottom line = reduce variable cost of care
Total cost of care: enables bidding on contracts
What is the cost of caring for CLI in a given
population ?
Reducing variability of care = physician involvement
How do we reduce cost ?
For endovascular procedures:
Supply cost
For open surgical procedures
LOS
Reducing variability in practice
There is a financial incentive for physicians on
production-based compensation towards Endo
Physician involvement is key
Conclusions
The cost of caring for patients with CLI is substantial
Open surgical repair: LOS drives initial cost
Endovascular therapy: Device cost drives intial cost
At one year, endovascular therapy is less expensive
Need for reintervention ~ 20% for both groups
At two years, surgical bypass becomes less expensive
Fewer hospitalizations for re-intervention
