Shadow Health and Social Care Joint Board arrangements
and Third Sector Interface (TSI) contact details

Introduction
This document was started by the ALLIANCE to help inform third sector
organisations about the ongoing process of health and social care integration
locally. It contains details about the key people and partnership structures
developing in each local authority area of Scotland. CCPS has begun to add further
information that may be useful to CCPS members and others about the relevant
contacts and key people within the TSIs in each area. It is very much a work in
progress that we intend to update regularly and welcome any suggestions you may
have regarding amendments, updates or additions.
Each area is currently deciding its approach to integration. The Scottish Government
has produced a timeline setting out the principle milestones between now and April
2016. More information about the implementation of the Public Bodies (Joint
Working) (Scotland) Act 2014 can be found on the Scottish Government’s designated
webpage.
The first set of affirmative regulations were laid in Parliament on Friday 3 October
2014. They include the prescribed content of the integration scheme; functions that
must be delegated by local authorities; functions that must and may be delegated by
health boards; and the national health and wellbeing outcomes. There is also a
supporting note with further explanation of the delegated functions. These
regulations are expected to take effect in December 2014.
In addition, each new partnership is expected to have agreed an integrated care
fund plan by 12 December 2014. This involves the spending of a proportion of the
£100M allocated by the Scottish Government to each area to support delivery of
integration, including the shift towards prevention. The Scottish Government
integrated care fund guidance builds on the experience of the Reshaping Care of
Older People, including requiring sign-off by third sector representatives. Work is
ongoing among CCPS and its third sector partners to support engagement with this
process locally.

Aberdeen City
Aberdeen has adopted the body corporate model of integration.
A Transitional Leadership Group has been set up comprising of membership from
elected members of the Council, non-Executive Directors of NHS Grampian alongside
clinical and professional leads as well as representation from stakeholders across the
third and independent sectors. This is split as follows:







6 elected members of Aberdeen City Council;
6 non-executive board members of NHS Grampian;
Director of Social Care and Wellbeing (Chief Social Work Officer), Aberdeen
City Council;
General Manager, Aberdeen CHP;
1 clinical lead NHS Grampian;
The Third Sector, represented by Aberdeen Council of Voluntary
organisations, on behalf of people who use support and services and citizens;
and
2 Trade Union representatives.
Judith Proctor has been appointed Chief Officer of the Transitional Leadership
Group.

final formation of a formal Integrated Joint Board.

Aberdeenshire
Aberdeenshire has adopted the body corporate model of integration, establishing
Transitional Leadership Group has been established comprising Aberdeenshire

make recommendations to both parent organisations on the design and model of
integration for Aberdeenshire.

Argyll and Bute
Argyll and Bute have adopted the body corporate model of integration, having
endorsed a joint report in April 2014.
A Shadow Health and Social Care Partnership has been developed to provide joint
support and direction to the establishment of the integrated arrangements by April
2015. It has been recommended that the board adopt a membership approach of 4
councillors and 4 Health Board members. This will meet for the first time in
September 2014.
A new chief officer, Christina West, currently Director of Operations for Argyll and
Bute Community Health Partnership for NHS Highland, will take up the post shortly
in anticipation of the new integrated organisation being established from 1st April

Clackmannanshire
Clackmannanshire Council, Stirling Council and NHS Forth Valley Health Board will
work together under a joint body corporate approach to integration.
Clackmannanshire Council agreed in June 2014 that the current Partnership Board
(existing between Clackmannanshire Council, Stirling Council and NHS Forth Valley
Health Board) will form the basis of a Shadow Integration Board and proposals will
be put to that Board when it meets on 9 September, 2014, to take this forward. This
will include requirements for expanded membership as set out in the Regulations.
A full timeline of the integration process has been submitted by Officers for
approval. This details that recruitment of Chief Officer will start by March 2015 and
be complete by July 2015.

This is expected to operate as follows:
The Dumfries and Galloway Community Health and Social Care Partnership Board
was established on 26 June 2008 in order to further develop and implement a joint
vision and joint working for Health and Social Care Services in Dumfries and
Galloway between the NHS and the Council. The current board membership, seven
from both the Council and NHS Board, exceeds the minimum requirement and
continuity of this approach is felt to be beneficial and effective in ensuring the
delivery of this policy development.

The Shadow Partnership will be strengthened by additional membership of the

As of June 2013, agreement had still to be reached on carer/user
representation. The Shadow Partnership will be chaired by Mike Ash, who previously
chaired the Community Health Partnership sub-committee.
The Shadow Partnership’s remit will include developing five specific workstreams
Finance and IT; Governance; Outcomes; Strategic Commissioning; and HR and
Workforce Development.
The Partnership will adopt the body corporate approach, appointing a Jointly
Accountable Officer to co-ordinate and drive the work of the Shadow Partnership
and manage the transition to the Health and Social Care Partnership.

The first meeting of the Shadow Integration Joint Board was held on 23 June and
discussed progress to date, joint response to the regulations and orders, stakeholder
representation and the development of the shadow board. The next meeting of the
Shadow Integration Joint Board is on 22 September 2014.
A workshop on locality planning was held for health, social care and community
planning staff on 24 June and further events are planned for October and November
2014 with a wide range of stakeholders, including the third sector and people who
use support and services/unpaid carers.
Colleagues from across Glasgow attended an event on 26 August to discuss a joint
strategic plan for the city and how planning structures may need to change in the
future.

Highland
On 1 April 2012, Highland Council and NHS Highland Health Board integrated
elements of their health and social care services. In December 2010, the Council
and the Health Board began a 15 month process which would lead to the integration
of Adult Community Care Services and Children’s Services across the Highland
Council area.

The model
Highland has adopted a “Lead Agency model”. Essentially this means that a lead
agency is responsible for the delivery of a service and management of staff and
budgets. In this instance, adult social care is delegated to the Health Board and
children’s community health services are delegated to the Local Authority. Both

organisations retain accountability, supporting committee structures need to exercise
scrutiny and reporting and performance measures must be based on agreed
outcomes.
In practice, this means that the Chief Executive of the Health Board is accountable
to the Council and Health Board for delivery of adult services and the Chief Executive
of the Local Authority is accountable for delivery of children’s services.
The model adopted in Highland places responsibility for delivery in the hands of the
Lead Agency and this allows for that flexible use of resource focussed on the
outcomes. The agreed performance management framework and performance
indicators ensure that both organisations can be held to account and evidenced
through the outcomes achieved.
Staffing implications
This decision had the following staffing implications:

The transfer of approximately 1,500 Social Care Staff and around £90million
to NHS Highland to support the redesign of adult care

In Children’s services around 250 staff and £8million would transfer to
Highland Council to enable redesign across Social Care, Education and Health
The ethos
The ethos of the integrated system is to achieve the best possible outcomes through
a person centred and enabling approach that anticipates and prevents need as well
as reacting to it.
“We will improve the quality and reduce the cost of services through the creation of
new, simpler, organisational arrangements designed to maximise outcomes and the
streamlining of service delivery.”

Budgets and financial implications

In the first financial year, the contribution payable by Highland Council to NHS
Highland was predicted to be around £88.3m.

In respect of Children’s services, the financial contribution from NHS Highland to
Highland Council in the first financial year was predicted to be around £8.1m.

Midlothian
A shadow partnership board was formed in January 2013 to “deliver desired
outcomes for older people as soon as possible and achieve the explicit aim of adult
health and social care integration by 2014.”

Officers in attendance will include clinical leads from health and social work;
integration leads from health and the council; and a housing representative
In the Board’s first year the Chair would be held by an NHS Lothian member whilst
the Vice Chair would be the Cabinet Portfolio Holder for Community Care. These
positions will alternate annually.
The Board will meet on a 6 weekly basis and report regularly to the Community
Health Partnership (CHP) and the Council. In respect of issues outwith the delegated
authority of the CHP, the Board will report to NHS Lothian.
Midlothian has adopted the body corporate approach – and one of the first roles of

In September 2013, the following clarifications on transitional arrangements were
given to elected members of North Ayrshire Council.
 Elected member representation on the new Transitional Integration Board.
 The board will comprise 4 x Elected members and 4 x Health Board members.
These members will all have voting rights.

 Agreement on which services will be within the partnership.

 Hosting arrangements.
 Role of Policy Boards in relation to the Transitional Integration Board. It was
agreed that over the next 6 months discussion will take place in relation to
the role of the Health and Social Care Policy Board.
 The Joint Accountable Officer should be appointed by October 2013. At the
moment it is anticipated that this will be an internal appointment between
NHS Ayrshire and Arran and North Ayrshire Council.

Orkney Islands
Orkney Health and Care is a partnership between Orkney Islands Council and NHS
Orkney. Orkney Health and Care is Orkney’s local Community Health and Social
Care Partnership delivering health and social care services under an integrated
management structure. The Partnership became operational on 1 April 2010.
From September 2010 the Chief Executives of both organisations have taken on a
joint role to provide the on-going leadership, direction and co-ordination at both a
strategic and day to day operational level.

The Partnership has established a Joint Commissioning Strategy (2012-2022) which
details the services to be delivered through the health and social care partnership
working arrangements.

The full range of social services previously provided by Orkney Islands Council, and a
number of services previously provided by NHS Orkney are now under the joint
management of a single director, the Chief Executive of NHS Orkney, accountable to
the Council and NHS Orkney through the Board of Orkney Health and Care.
Orkney Health and Care is not responsible for acute hospital services including acute
medical and surgical services. The Partnership works very closely with these
services to avoid delayed discharges, reduce unplanned admissions and to promote
a joined up approach to health and care.
Orkney Health and Care Partnership Board consists of:


four elected Councillors
four NHS Board members
Their role is to oversee and scrutinise the work and on-going development of the
Partnership, including performance management and consideration of policy and
strategy.
The Orkney Health and Care Committee
Its members include all of the professions covered by Orkney Health and Care,
clinical input from acute services, child health and mental health, a carer
representative and a representative from the public partnership forum to ensure that
its advice is grounded in practice.
The Committee is chaired by an elected member of the Council and meets to secure
involvement of staff, service users and carers in governance and decision making for
Orkney Health and Care. The Committee has responsibilities to advise on areas such
as service redesign, planning, prioritisation and performance, operating alongside a
range of planning and working groups.
The Public Partnership Forum also meets regularly and provides a formal focus for
third sector and community engagement. The Public Partnership Forum provides an
advisory role which is complemented by specific broader consultation and
engagement processes on any service or policy changes.

Clackmannanshire Council, Stirling Council and NHS Forth Valley Health Board will
work together under a joint body corporate approach to integration.
At its meeting on Thursday 26th June 2014 Stirling Council agreed to
recommendations adopting an integrated model of governance for health and social
care.
The model will enable local partnerships, including the health board and local
authority, to deliver outcomes more effectively. It will also enable the Council to take
forward legislative requirements.
The integration will be based on a joint approach with Clackmannanshire Council
that is consistent with the two councils' shared Social Services.
