Grant Objectives
• Target Population - 300 HIV+ individuals in
Norwalk and the surrounding communities who
need dental care.

Promote importance of oral healthcare for individuals with
HIV/AIDS to providers of medical and case management
services in the community.
Significantly reduce and control dental disease in target
population.
Utilize resources to encourage adherence to appointments and
treatment plan.
Educate patient population on oral health self care
Publication and Dissemination.
