Its quality health care services are available to all, and particularly to those who cannot gain access to such services elsewhere. The Community Health Center takes leadership in promoting interagency cooperation. It is based on consumer control and is committed to ensuring human rights and respecting human dignity; as such, it strives to be a voice and vehicle for social change. Grant Objectives • Target Population - 300 HIV+ individuals in Norwalk and the surrounding communities who need dental care. • • • • • • Eliminate Barriers to dental care for target population. Promote importance of oral healthcare for individuals with HIV/AIDS to providers of medical and case management services in the community. Significantly reduce and control dental disease in target population. Utilize resources to encourage adherence to appointments and treatment plan. Educate patient population on oral health self care Publication and Dissemination.
