JOB PURPOSE
To successfully support the people you work for to take control and shape change
in ways that are meaningful for them. To lead in the planning and coordination of
support with the people you work for, their families and circle of support to assist
each individual to live a rich, valued and inclusive life. Promote active citizenship
and develop the leadership skills of the people we work for, their families and
circle of support to enable them to have the power, influence and responsibility to
make decisions.

KEY FOCUS AREAS
3.1
Outcomes

Develop and promote ‘Circles’ of natural support for the people you work for.

Encourage the people you work for to develop contacts, relationships and links
in their local community.

Promote opportunity for the people you work for to ‘shine’ by recognising their
strengths, gifts and skills and encouraging them in making a meaningful
contribution to their community, education, employment and relationships.

Ensure the development, implementation and review of Outcome Based
Support Plans for the people you work for.

Lead with the process of planning and ensure that this happens for the people
you work for, on at least an annual basis.
3.2
Management

Assist the people you work for, their family members and circle of support to
recruit and induct team members, who will support the individual to achieve
specific outcomes.

Assist the people you work for, their family members and circle of support to
oversee education opportunities, mandatory and specific training needs and
SSSC registration requirements for team members and ensure that these are
met.

Assist the people you work for, their family members and circle of support to
facilitate team peer support, on at least a quarterly basis.

Encourage team members to develop their creativity and problem solving skills
by always considering unpaid alternatives in the day to day support of the
person they work for.

Hold a vision for the people you work for and encourage team members to
aspire to more by adopting a risk enablement approach to risk management.

Hold responsibility for the development and regular updating of the people that
you work for’s Working Policy as per minimum standards

Have an active involvement in performance management and investigations

and disciplinary procedures.


Adopt a ‘coaching’ management style to your practice.

Support the people you work for at multi agency reviews and external meetings
and events.
Quality Assurance

Assist the people you work for, their family members and circle of support to
review the development and evaluation of their support and Outcome Based
Support Plan.

Have positive, regular and direct contact with the people you work for, their
family members and circle of support to ensure clear communication and to
promote involvement in the development of a person’s Outcome Based
Support Plan utilising their own personal on line planning tool, iplanit.

Spend time with the people you work for and where necessary with team
member(s) to review practice and monitor progress towards a person’s
outcomes.

Ensure that the health and well being of the people you work for is maintained
and any reviews, health checks are attended, acted upon and recorded.

Provide relevant information and written reports on impact, participation and
outcomes for the people you work for, their family members and circle of
support, C-Change Board and regulatory agencies.

Maintain awareness of key developments of Personalisation in Scotland and
the UK.
3.4 Finance

Assist the people you work for, their family members and circle of support to
manage their Individual Service Fund and use it to meet the proposed
outcomes detailed in their plans.

Facilitate the provision of an accessible quarterly statement on the Individual
Service Fund for the people you work for, their family members and circle of
support.

Support the people you work for, family members and circle of support to take
responsibility for financial transactions as detailed and agreed in their Social

Contract and ensure all necessary records are accurately maintained for
accountability.
Undertake any other reasonable duties as directed

TERMS AND CONDITIONS

While it is not envisaged that sleepovers will form a part of this role, the post
holder will be responsible for the emergency On Call for the people they work
for and may in some circumstances require to provide night time support

Although the C-Change base is located in Glasgow, regular travel throughout
Glasgow and surrounding areas may be an expectation of the post.

The post holder will work with between two and five people, who may be
geographically linked.

