DESCRIPTION
The purpose of the TEAM ACHIEVEMENT award is to recognize and reward teams who
work toward a common goal. Together, these individuals demonstrate commitment to
success through excellent and consistent performance.
What’s a team? Teams must consist of two to 12 members. Nominations for crossdepartmental teams are encouraged (i.e., members don’t all have to be from the same
department). What is most important to the selection panel is that the nomination
demonstrates the active participation of each and every team member towards the
achievement of common goals.
HOW TO SUBMIT
 Nomination forms must be submitted by email ([email protected]) with input from
at least two peers or colleagues no later than Monday, December 12, 2016, at 5
p.m.
 Remember: Winners are determined solely on this nomination form. Please be as
specific as possible when describing projects/accomplishments
HOW THE AWARD WORKS
 Please review all nomination criteria listed in the brochure

Employees may nominate their peers and/or supervisor but downward nominations
are not accepted

Frontline workers and middle managers may be nominated
 No self-nomination or nomination of family members
 All information provided will be verified by contacting the nominee’s supervisor
 Achievements must occur during the time period from August 2015 to September
