Member Guide
For Medica Choice CareSM – Medical Assistance (MA)
If you do not have access to a vehicle, Medica will help you
schedule transportation to and from health care visits. This
guide tells you how to schedule a ride, and more information
about your transportation benefits.
Where can I get a ride?
You can get rides for these types of visits:

How do I cancel or change a ride?
ƒƒ Call Provide-A-Ride as soon as possible
ƒƒ During non-business hours, you must call the transportation company
What do I need to know about my pick-up and return rides?
Pick-up Ride
ƒƒ Pick-up locations can only be at your home, school, work or a shelter
ƒƒ Pick-up time is 45 minutes before your appointment time; earlier if needed
How do I set up a ride?
ƒƒ The driver will wait 5 minutes at the pick-up location
ƒƒ Call Medica Provide-A-Ride at the number on the back ƒƒ The driver will give you a return slip with the phone of this page
number to call for the return ride when your visit is done
ƒƒ Tell them you need a ride
Return Ride
ƒƒ Have your information ready:
ƒƒ Call the number on your return slip when your visit is done
ƒƒ Medica ID number or Social Security number
ƒƒ Identify yourself as a Medica member
ƒƒ Date of birth
ƒƒ Let the driver know the door where you will be ƒƒ Doctor or clinic’s full name and address

Types of Transportation
ƒƒ Return rides may take up to 30 minutes; up to
60 minutes during rush hour or bad weather
ƒƒ Bus Rides will be provided to members who live on a bus
ƒƒ The driver will wait 5 minutes at the return ride location
line. Call at least three days before your appointment.
ƒƒ Taxi or volunteer driver program rides will be provided for

For urgently needed rides, call Medica CallLink Nurse Line at
ƒƒ Minneapolis/St. Paul metro area, call at least 1-800-962-9497 (toll-free). TTY users call the National Relay Center

areas or at all times.
Urgent Care and Emergency Room visits
If you call for a ride to urgent care or the emergency room,
Medica will confirm with the Medica CallLink® Nurse Line that
the care you need is urgent. If it is, a ride will be provided. If the
care you need is life-threatening, we will have you call 911 for
help.
Reasons a ride may be denied
ƒƒ The ride was to a place or for a service your health plan does not cover
ƒƒ You do not have the doctor or clinic’s full name and address
ƒƒ You do not have a referral for the appointment when it is required
ƒƒ Your coverage with Medica is not active on the date of the visit
ƒƒ You did not call early enough to schedule the ride
ƒƒ The location of your appointment is more than the State’s distance limit for a ride
ƒƒ You have access to a working vehicle

Please know that abusive behavior including the use of
profanity, not showing up for your ride and other misuses of your
transportation may result in a warning and change in available
ride options.

American Indians can continue or begin to use tribal and Indian
Health Services (IHS) clinics. We will not require prior approval
or impose any conditions for you to get services at these clinics.
For enrollees age 65 years and older this includes Elderly Waiver
(EW) services accessed through the tribe. If a doctor or other
provider in a tribal or IHS clinic refers you to a provider in our
network, we will not require you to see your primary care provider
prior to the referral.
