Medica Provide-A-RideSM Member Guide For Medica Choice CareSM – Medical Assistance (MA) If you do not have access to a vehicle, Medica will help you schedule transportation to and from health care visits. This guide tells you how to schedule a ride, and more information about your transportation benefits. Where can I get a ride? You can get rides for these types of visits: Medical Dental Mental Health Chemical Dependency Pharmacy Durable Medical Equipment Special Medical Transportation Call Medica Provide-A-Ride to get a list of covered providers. How do I cancel or change a ride? Call Provide-A-Ride as soon as possible During non-business hours, you must call the transportation company What do I need to know about my pick-up and return rides? Pick-up Ride Pick-up locations can only be at your home, school, work or a shelter Pick-up time is 45 minutes before your appointment time; earlier if needed How do I set up a ride? The driver will wait 5 minutes at the pick-up location Call Medica Provide-A-Ride at the number on the back The driver will give you a return slip with the phone of this page number to call for the return ride when your visit is done Tell them you need a ride Return Ride Have your information ready: Call the number on your return slip when your visit is done Medica ID number or Social Security number Identify yourself as a Medica member Date of birth Let the driver know the door where you will be Doctor or clinic’s full name and address waiting Types of Transportation Return rides may take up to 30 minutes; up to 60 minutes during rush hour or bad weather Bus Rides will be provided to members who live on a bus The driver will wait 5 minutes at the return ride location line. Call at least three days before your appointment. Taxi or volunteer driver program rides will be provided for Rides needed when the Medica office is closed members who do not live on a bus line. For urgently needed rides, call Medica CallLink Nurse Line at Minneapolis/St. Paul metro area, call at least 1-800-962-9497 (toll-free). TTY users call the National Relay Center one business day before your visit at 1-800-855-2880 (toll-free), and ask for 1-800-962-9497. Outside the metro area, call at least five Medica CallLink Nurse Line is open 24 hours a day, seven days a business days before your visit week. An after-hours urgent ride may not be available in all areas or at all times. Urgent Care and Emergency Room visits If you call for a ride to urgent care or the emergency room, Medica will confirm with the Medica CallLink® Nurse Line that the care you need is urgent. If it is, a ride will be provided. If the care you need is life-threatening, we will have you call 911 for help. Reasons a ride may be denied The ride was to a place or for a service your health plan does not cover You do not have the doctor or clinic’s full name and address You do not have a referral for the appointment when it is required Your coverage with Medica is not active on the date of the visit You did not call early enough to schedule the ride The location of your appointment is more than the State’s distance limit for a ride You have access to a working vehicle Medica Choice CareSM (MA) and Medica MinnesotaCare members Call Medica Customer Service at 952-992-2322 or 1-800-373-8335 (toll-free) Medica AccessAbility Solution® (SNBC), Medica DUAL Solution® (MSHO) and Medica Choice Care MSC+ members Call Medica Customer Service at 952-992-2580 or 1-888-347-3630 (toll-free) Please know that abusive behavior including the use of profanity, not showing up for your ride and other misuses of your transportation may result in a warning and change in available ride options. If a doctor or other provider in a tribal or IHS clinic refers you to a provider in our network, we will not require you to see your primary care provider prior to the referral.
