WAIVER, RELEASE AND HOLD
HARMLESS AGREEMENT
The undersigned, ____________________________________, (“Participant”) is a voluntary participant in
the Sports Plus Inline Hockey Program. The team will play on ice rinks located at Sports Plus, 10765
Reading Road, Cincinnati, Ohio 45241.
The Participant hereby releases and forever indemnifies and holds harmless MOI Operations, Inc., Martini
On Ice, LLC and/or any of its affiliated organizations, for any injury or damage that he/she may sustain as a
result of his/her participation in the Sports Plus Inline Hockey Program. The Participant fully understands
that play is at his/her own risk.
The Participant has read and understands the rules and regulations posted by MOI Operations, Inc. dba
Sports Plus, and agree to abide by such rules and regulations, including but not limited to all equipment
requirements. Participant understands that any violation of such rules and regulations is cause for
immediate removal of Participant from the rink and/or Sports Plus facility.
The Participant acknowledges that he/she has carefully read this Waiver, Release and Hold Harmless
Agreement and understands its contents. Participant, by his/her signature, acknowledges that he/she is 18
years of age or older and is authorized to sign this Waiver, Release and Hold Harmless Agreement, and that
this is a full release of liability by Participant and that Participant has executed this Waiver, Release and
Hold Harmless Agreement willingly.
