INDIVIDUAL REGISTRATION GUIDELINES
All coaches, assistant coaches, team managers, and volunteers must meet specific USA Hockey, MidAm District, and
Pennsylvania state requirements. The list below is detailed but not intended to be all-encompassing. The specific
requirements needed for each team, association, school district, and/or state may vary. This document simply
assists all of these individuals in completing the registration process and applicable requirements.

USA HOCKEY REGISTRATION
Deadline: Prior to any team participation
Who: All coaches, team managers, and volunteers
What: Each individual must register online with USA Hockey
When: This must be completed annually
Cost: Coaches = $45.00; Managers/Volunteers = free
How:
1. Go to www.usahockeyregistration.com and complete the registration process
2. Provide your registration number to your Registrar
SIGNED USA HOCKEY PAPERWORK
Deadline: Prior to any on-ice participation
Who: All coaches, team managers, and volunteers
What: Each individual must complete and sign required forms
When: This must be completed annually
Cost: free
How:
1. Print the USA Hockey Code of Conduct and sign

2. Print the USA Hockey Consent to Treat, complete, and sign

3. Submit both signed forms for your Team Book
MID-AMERICAN DISTRICT SCREENING
Deadline: Prior to any team participation
Who:
 All coaches & managers
 All volunteers with direct and/or routine access to children
 Note: required regardless of other clearances a person may possess
What: A background screening processed online through the MidAm vendor
When: MidAm screening is valid for 2 years
Cost: $25
How:
1. Go to www.midamhockeyscreen.com
2. Register/log in and enter required information
3. Complete and fax the DMV authorization letter to the vendor (EBI)
4. Allow time for screening to be processed
5. To return and print results:
a. Log into the MidAm Screening web site
b. Choose the option to View background screening results page
c. Click the Blue Ribbon icon under the Candidate Status for the letter
d. Click the Green icon under the Candidate Status for the report
e. Save the MidAm letter and print for your Team Book
SAFESPORT TRAINING
Deadline: Prior to any team participation
Who: All coaches, managers, and locker room volunteers
What:
 Initial SafeSport training module (90 minutes) for all new personnel
 30-minute SafeSport refresher module for those with expiring certification
When: SafeSport Certification is valid for 2 years
Cost: free
How:
1. Visit www.usahockey.com/safety and view the correct module
2. Print the completed SafeSport Certificate for Team Book
HEADS-UP CONCUSSION TRAINING
Deadline: Prior to any team participation
Who: All coaches residing in Pennsylvania and West Virginia
What: 30-minute online training module
When: This must be completed annually
Cost: free
How:
1. Visit www.cdc.gov/concussion/HeadsUp/Training/ and view the training module
2. Print the completed Heads-Up Concussion Certificate for Team Book
COACHING EDUCATION PROGRAM (CEP) CERTIFICATION
Deadline: By December 31st of current season
Who: All coaches
What: Coaches must do a clinic or recertification
When:
 This must be done each year until obtaining Level 3
 Level 3 is valid for 2 seasons, then can be recertified twice (each valid for 2 seasons)
 No further clinics/recertification required once obtaining Level 4
 Maximum of one clinic/recertification per year, regardless of what level
Where: Clinics are scheduled at various locations within the region
Cost: Clinics = $40; Modules = $10 each
How:
1. Check current CEP status via USAH database

2. Obtain required certification for current season via one of the following:
a. Sign up for appropriate level CEP Clinic and attend, as scheduled

b. Complete online recertification module for Level 3

COACHING AGE-SPECIFIC MODULE (ASM) REQUIREMENT
Deadline: By December 31st of current season
Who: All coaches
What: Coaches must complete the ASM for the age division(s) being coached
When: Once completed, the coach does not have to repeat that particular ASM again
Cost: Each online module is $10.00
How:
1. Check current CEP status via USAH database

2. Complete appropriate ASM Module, as needed

NEW PENNSYLVANIA STATE ACT 15 LAW
Deadline:
 For all coaches and managers new to their programs – before any team participation
 For all returning coaches (paid) – by December 31, 2015
 For all returning coaches (unpaid/volunteer) – by July 1, 2016
 For all new volunteers – before any team participation
 For all returning (active prior to 8/25/15) volunteers – by July 1, 2016
Who:
 All coaches and managers residing in the Commonwealth of Pennsylvania
 All volunteers who have direct and/or routine access to children and are PA residents
What: 3-part background check of criminal history
When: Process is good for 5 years
Cost:
 State Criminal History Record = $10
 Child Abuse Clearance = $10
 Federal Criminal History Record = $27.50
How:
1. Complete State Criminal History Record (PA state police)

2. Complete Child Abuse Clearance (Dept of Human Services)

3. Complete Federal Criminal History Record/Fingerprinting (FBI)

b. If a volunteer has lived in PA for 10 years or been a volunteer for the organization before Dec 2014, then
he/she can sign a waiver in lieu of the FBI clearance

4. Confirm with your association and/or school district that all clearances are satisfied
OTHER POSSIBLE REQUIREMENTS
Please check with your association, your school district, and any other governing affiliate (such as Catholic Diocese)
for other possible screenings, certifications, and/or training.
