Guidance on completing the Application Form for tutors on SHIPSAN ACT Joint Action training courses
1. Please only submit the approved application form. Do not send CVs or supporting documents.
2. Please submit type-written forms, in English.
3. The application form is in Word 97-2003 format. In the column “supporting detail” please give the number of years experience, title of
degree, hours of training experience, etc. There is no restriction to how much information you can include, but please keep it relevant to the
criterion. Irrelevant cut-and-paste from CVs will be discarded.
4. There are separate application forms for different modules of the training course. These are; Communicable Diseases and Outbreak
Management, Food Safety, Housekeeping, Pest Management, Waste Management, Water Safety and Water Supply Plans, Backflow
Prevention, Technical Requirements for Water Supply System, Recreational Water Safety, IHR Legal Framework and Drinking Water Chemistry
and Related Health Effects. Prospective tutors who wish to apply to train more than one module should complete a separate form for each
module for which they wish to be considered.

6. The deadline for submission of application forms for the initial training modules is 15 July 2013.
