30 days of payment date. Include players name on ALL cheques.

Fees are due to be paid in full by Aug1/2017 (after this date a late payment fee of $100.00 will be
applied)

$175.00 (registration on or after Aug 1st,2017 is $200.00)
Family Discount – take 50% off the youngest child(ren) in a family with 3 or more in same family, only applies to
base registration fees (not try out fees) and is applied to the lowest registration rate.

Goalie discount –PCMHA will provide a Goalie discount for players registered as full-time goalies and who provide
their own gear; in Atom the discount will be 50% and from Peewee – Midget the discount will be 35%. The association will
provide goalie equipment for players registered in H1 – Peewee and no Goalie discount will be offered.

*NOTES**:
Rep Tryouts:

No refunds will be issued after August 1, 2017. Refunds must be requested by email to the Registrar. It is
recommended that you register for rep tryouts with your initial registration as the fee increases to $200.00 on August,
1 for all late rep registrations. Rep tryouts are tentatively scheduled to begin the last week of August. Players with
extenuating circumstances should contact the President directly in writing to request an exemption for this week

 Players will not be permitted on the ice if:
1. There are Any REGISTRATION payments/deposits still outstanding at the time of fist ice
allocations
2. Proof of residency has not been received
 Payments and proof of residency can be mailed to PCMHA PO Box 44, Port Coquitlam V3C 3V5
 Payment is accepted by cheque, money draft, or pay pal only. Waitlisted players are asked to provide
one cheque for the full amount – dated September 1, 2017.
Proof of Residency (required every year)
Register online, then send in proof of residency, (your registration will only be considered complete when this
information is received).
You MUST provide two pieces and they MUST be:
(1) Property tax assessment or Lease agreement (lease agreement must be all pages)
(2) Utility bill i.e. BC Hydro or Fortis BC. Shaw or Telus invoices are acceptable as long as it is for
cable/internet. Phone bills are not accepted.
Volunteer Policy

Port Coquitlam Minor Hockey is a non-profit organization that relies on volunteers for all association
sponsored events. Parents/caregivers of each child can help by volunteering for a variety of positions or
duties, within the association which can be coaching, managers, HCSP, or other areas where support is
required.

When registering, please include a $150 "Volunteer Duty Deposit Cheque" made out to PCMHA (dated
April 1, 2018) which is destroyed, or returned if requested, once you complete a volunteer shift; if no
volunteer hours are complete, your cheque will be cashed.
o Volunteer shifts are one per family and the duration is 3 hours per season; opportunities are
communicated via email by [email protected] or by the Registrar

o Team duties such as score-keeping, clock duties or music during games are excluded from this
policy.

Cancellation Policy:

Cancellations received before August 15th refund less $100 admin fee, September 30th
refund less 50% of base registration, October 15th no REFUND-exceptional circumstances
may be reviewed by the Executive Directors. It is the responsibility of the player/family to fill
out the Refund Policy Form found on the website www.pocominorhockey.com**
