Goal Accomplishment (Note: If you used this tool
in the fall and only need to update your postassessment data please refer to step 14.)
4. Click on the download button for the Student Score Sheet template
and save it to your desktop.

5. The Student Score Sheet template can alternatively be downloaded
from the Schoolnet homepage. Open Firefox and enter the following

2. Roll the cursor over the Resources tab. From the scroll down menu
select Student Learning Objectives (SLOs).

6. Under TEI Related Resources click on Sheet to download the Student
Score Sheet template and save it to your desktop.

select Overview Documents.

7. Open up the file.
10. Fill in ALL blue columns. Enter baseline assessment date, baseline
assessment score, baseline assessment max, points, growth
target, outcome assessment date, outcome assessment score, and
outcome assessment max. points.
11. Roll the cursor over Eligible Based on Attendance, and select Yes
or No from the drop-down menu.
8. Enter Teacher Name and Subject Area next to the Student Score
Sheet header.
12. The spreadsheet will automatically populate ALL purple columns
along with the summary calculations in the yellow box.
9. Enter student roster. Include only those students eligible for SLO
calculation. (These students must have met the 85% attendance
criteria.)

15. Fill in ALL remaining blue columns. Enter outcome assessment
date, outcome assessment score, and outcome assessment max.

19. If you are using this spreadsheet to document your SLO results,
then this spreadsheet must be uploaded as an artifact in the SLO
goal-accomplishment window. Follow the instructions to attach an
artifact included in the Submitting a Goal Accomplishment SLO
QRC available in the Schoolnet homepage and the SLO website in
Curriculum Central.
16. Roll the cursor over Eligible Based on Attendance, and select Yes
or No from the drop-down menu.
