2015 Donation Request Stipulations:
In exchange for a donation of services to be provided for your event we ask the following to be agreed
to in writing:
-ALL advertising and marketing material of said event must include the Nightlife Entertainment logo and
phone number/contact info in an agreeable length of time (prior to the event) in all means of
advertising used for your event. Once you begin advertising/promotion of your event, Nightlife’s logo,
name and contact info must be promoted as a donor/sponsor, the means of promotion are including but
not limited to all the listed: (all signs, banners, table top ads, event flyers, posters, newspaper/magazine
ads, press releases, brochures, rack cards, postcards/mailings, news broadcasts, event Facebook pages,
weekly or bi-weekly posts on your website and social media pages, on an event website(s)/any other
social media sites used for said event, event apparel, t-shirts, etc.) Promotion of Nightlife Entertainment
must begin immediately in your advertising/promotion efforts and no less than a month before event.
-Allow Nightlife Entertainment to promote said event in all its advertising outlets as a sponsor and donor
before, during and after event.
-Permission is granted to all employees, representatives and volunteers of Nightlife Entertainment to
promote our business through use of signage, public announcements and displays of business cards at
said event.
-You give permission and release of liability to Nightlife Entertainment to use your pictures and
video/our pictures and video of said event for future advertising purposes.
-You/head of your organization sign a copy of an “in kind” donation statement letter or invoice for tax
purposes equaling the amount of donated services with all applicable fees.
-Use the general document planning guide given to me by the Dj/Emcee assigned to my event and have
that paperwork turned in no later than 2 weeks before said event.
-Agree to release Nightlife Entertainment, its employees, representatives and volunteers harmless of all
liability related to said event/property for damages, financial or legal purposes.
-A written testimonial is given 14 days after donation of service.
For donation of a raffle item, a monetary gift or similar “gift” idea for an event:
-Person in charge of event requesting the raffle item or monetary donation will need to supply a
minimum/maximum dollar amount expected in writing to be donated in request form supplied by
Nightlife Entertainment.
-All item(s) donated will have a donation branding on the wrapping or outside of package detailing the
Nightlife logo/contact info on labeling or card and should be announced verbally as “Nightlife
Entertainment- Music for all Occasions” as the company donating the item during raffle.
-Provide the tax id # of your organization/event or sign off on an “in kind” donation letter verifying
Nightlife Entertainment donated the said item in the said amount for donation purposes.
-Give Nightlife Entertainment permission to use the name of the event and video or photos of the item
donated for both online and print advertising, “shout out”, etc.

Donation requests must be received as far prior to the event as possible. We will
reject those requests that are less than 30 days away. Nightlife Entertainment
donates DJ/Emcee services, Sound services, Lighting and Up lighting services and
donations in monetary or gift requests at our discretion. Due to the volume of
requests received throughout the year, we are only able to do a limited number
events that fall within a number of factors regarding our annual donation budget,
availability, level of interest in event all at owner’s discretion. We will return with
a response to your request within 7 business days. Thank you for your interest in

Please remember if your event is chosen to receive donation of services, a list of stipulations
are asked to be agreed upon in exchange for services of no charge. (These stipulations are
found on our website, under the Donation Request page entitled “Donation Request
Stipulations”.) Please give your signature below and date this document, only if you agree to all
stipulations as written in that document.
I, _______________________________, (printed name), manager of event/person named in
charge as found on form above, have read all requested stipulations and fully understand what
is expected of me in order to receive donated services through Nightlife Entertainment on the
date named above. I agree to follow all of the written stipulations and work together with my
assigned Dj/Emcee in a timely matter for planning purposes. Should any of the above
stipulations not be completed, the owner of Nightlife Entertainment holds the right to cancel
donation of services scheduled to be performed or donated in part or full at his sole discretion
without notice. I also agree to photo/video release of my event and to hold Nightlife
Entertainment harmless of any damages or legal/financial liability not directly related to their
performances of services, equipment or actions of employees/volunteers.
