Instructions to log onto the Bear River High School Athletic Registration site. The instructions in
red are for those who haven’t already registered this year. They have to register each school
year! If they have registered this year they just need to start where the black writing starts.

FEES: Athletic fee = $70.00. Drug Testing consent $10.00
Click on “Register a New Account”
Click to Choose which “sport” and click on arrows “>>” to add to list on the Right.
Click “Step 2”
Complete Athletes information (*) are required fields. Choose Username and Password.
Click “Step 3”
Fill out parent information and choose drop down of Father or Mother on the left hand side of name.
Click “Step 4”
You will need insurance information. Be sure to Click “I am covered”.
Click “Submit Registration”
Click “Return to Login” and
Click “login” username and password.
Click on “Forms and Fees” tab. Your Sport should be auto filled for you. Double Click on the “Sport” in
the middle.
First time or new to this school: Athletes Click and Print “Form A” this form needs to be completed by a
physician.
Returning Athletes Click and Print “Form B” You need to fill this out Completely (does not need a doctors
signature). This form only needs to be filled out once per year.
Click on the following forms and PRINT them.
“Drug Testing”, only once each year “Tryout Check List”, “Sportsmanship paper, &“Master
Signature Form” must be printed off and filled in each time before you try out for the sport.
Read the following forms: (You DO NOT have to print them.)
“Parent and Student Consent Form”, “Code of Conduct and Media Publication Form”.
Click “I have read and agree to all the forms above”
Click “Update” this will save it!! Fill out the forms you printed COMPLETELY and return them to your coach or to
the Athletic Director (Coach Park)
