Mosborough After School Club is open to children from Mosborough Primary School only.
2.
Before the start of each new term we will require a booking form for the following term. Payments must be
made in advance of the sessions booked. If your child is not going to attend the club for whatever reason school
must be informed by 12 noon of that day. Due to the limited availability of spaces the full daily charge for the
session will be made.
3.
Payment can be made by cash or cheque (payable to Mosborough Primary School) and sent along with a booking
form. If payment is made in cash please ensure that the booking form is signed by both yourself and a member
of school staff to acknowledge the payment. The school cannot take responsibility for lost payments. Receipts
are available on request. If you are paying by cheque for both After School Club and Breakfast Club please make
two separate payments.

Short notice bookings will be accepted subject to availability of places and receipt of payment to secure the
booking.

A completed Registration Form and Health Registration Consent Form is required for each child attending the
club.

Children must be collected by 6:00 p.m. Late collection will incur a cost of £5 for every 10 minutes over, unless
this is due to adverse weather e.g. heavy snow. If parents or carers are going to be late they must inform the
After School Club staff.
7.
Children will be collected by After School Club staff from their classrooms at the end of the school day. If your
child attends an activity club after school a member of staff will collect them from the club.
8.
There will be an annual review of charges each April.
9.
Children will be provided with a light snack, served between 4:30 and 5:00 p.m. and a two week menu showing
the snack for each day will be available. Parents will be asked to make a selection for each day their child is
attending and return to the Play Leader to ensure the snack of choice can be provided.
10.
Parents/Carers are expected to provide up to date contact details to ensure they are contactable at all times
whilst their child is attending the After School Club. Staff must be informed of any changes to medical
information or contact details immediately.
11.
After School Club staff are unable to administer medication which has not been prescribed by a doctor.
However, staff are able to administer medication which has been prescribed by a doctor. Parents/carers will
need to complete a Health Registration and Consent Form.
12.
The After School Club is part of Mosborough Primary School. All relevant school and Local Authority policies
therefore apply. These include the school’s Safeguarding, First Aid, Administering Medicine and Health and
Safety policies, available on request
13.
Children are expected to behave safely and sensibly at all times. Unacceptable behaviour may result in the
child’s place in the club being withdrawn.
14.
The school reserves the right to change these terms and conditions with one month’s notice.
