INSTRUCTIONS
The purpose of the program is to recognize, honor, and publicize the accomplishments, expertise and
leadership of members of the PMI CMASS chapter who demonstrate superior performance, leadership
and execution of exemplary project management practices. In addition, PMI CMASS chapter would like
to promote the Project Management Profession and the value of the Project Management Institute (PMI).
1. The application should reflect project completion (Formal Closing as defined by PMBOK)
between January 1, 2015 and June 30, 2016.
2. Applications can be submitted by any one:
a. Self nominated by the Project Manager
b. Nominated by a company or manager
c. Nominated by a team member
3. In order to keep the applications anonymous, please:
a. Make sure the name of the Project Manager, Company and Team Members are only
listed on the cover page.
b. That throughout the application, the following replacements are used:
