YES Prep Athletics Student Survey
Each year, YES Prep Public Schools conducts an athletic program evaluation. These evaluations are conducted in
an effort to improve every aspect of our athletic programs. After each sport season has concluded, we administer a
survey to each athlete, in every individual sport. All surveys are kept anonymous. Please take a few moments to
complete your survey. Your participation in this process will provide valuable information which will be used to
make any necessary program adjustments.
Is your coach approachable?
Was your experience as part of this team a positive one?
Do you feel your coach is a positive role model?
Do you feel that your skill level improved from the beginning of the season to the end of the season?
Did your coach communicate well with you concerning schedules and policies?
Do you feel that your coach put in the extra effort to make the program successful?
Do you feel your coach has the knowledge of this sport for it to be successful?
Do you feel your coach was fair and consistent in his/her treatment of athletes?
Do you feel that the regular season schedule of your team prepares the team for post-season play?
Is your coach a good teacher of the game?
Did you attend all practices?
Did you attend all pre-season events (i.e., open house, meeting, etc.)?
Did you ever meet with the coach individually?
Did you enjoy the overall quality of coaching?
Where there any Effective or motivational techniques used by the coaching staff?
