Uniform Expections
If a student is out of uniform as the cross the front doors they will receive a mark/demerit and a
detention if it cannot be corrected.
YES Prep NF polo Monday-Friday with a belt
ALL outer layer must be YES Prep apparel. NO EXCEPTIONS!
Friday dress –If you do not have any marks or demerits then you may wear jeans and either a college
shirt or a YES shirt. NO RIPPED JEANS!
Uniforms are sold Tuesday and Thursday’s before school.

Cell Phone Expectations
Cell phones and electronic devices are not permitted. If taken up, parents will have to pay $10.





As of 9/22/14, there are only 2 school days left before students with no approved
application on file will be considered paid. All students without an “approved”
application on file by Wednesday, September 24 will roll over to PAID
status. Lunch will then be $3.00 per student. Once they meet the $15.00 meal
balance, students’ accounts will then be charged $1.00 for an alternate meal
consisting of a fruit and granola bar. Students will only be allowed 5 alternate
meals before the schools will then decide to have a conference with the parents
regarding the outstanding meal balance. **Please highlight Wednesday,
September 24 on your calendars**

AT&T is progressing towards servicing a complete 4G+ network. This means that service on phones that only
support 3G service (such as the iPhone 3 models) will deteriorate as we move forward. If you have an
iPhone3 (or another 3G network phone) and you have not gotten a new phone in the past year (you are
upgrade eligible), it is encouraged you follow the instructions below to use your phone upgrade:

anticipated that the demand on Apple will be very high, so you should anticipate long wait times for devices
to arrive (there will almost certainly be a large backorder queue).

below. Select Share.
Moving forward you will need to maintain your logs on your OneDrive. All Microsoft programs have the

Send Home State Accountability and SSI information <Action Required>

The state requires that campuses send home State Accountability Ratings. With the first round of report
cards, every student should also receive following: 1) the one-page summary TEA released document for
your campus, 2) an overview explanation document, and 3) SSI Brochures (to 8th grader families only).
Below are the required action steps:
1) Download the first two files from the State Accountability O365 site (snippet below), accessible via
the Analytics landing page, and pick up the SSI Brochures from HO.
2) Inside the explanation letter, change the yellow-highlighted text to the correct date and campus
contact information. This contact could be the Campus Testing Coordinator, a DoA/DoI, etc.
(whoever the families should contact if there are questions).
3) Print the explanation document front/back and the one-page campus summary
4) Work with Ops Manager and CTCS to ensure that the documents (two per student; three for 8 th
graders) are sent home with students’ A1 report cards

Homebound Services <Info Only>

Given the critical feedback of several stakeholders, the Student Support
Team is aware that there was a lack of clarity surrounding the General
Education Homebound process and staff responsibilities. After consulting
with teachers, administrators, and staff and weeks of research, we are
pleased to announce that we have we have revised the General Education
Homebound (HB) protocol. With the revision, our goals are to align with
Texas Education Agency (TEA) guidelines, streamline the process, and
provide effective support to all students. The revised protocol has a procedural flowchart, which clearly
outlines the process from start to finish, along with parent and staff functions. There are six updated YES
Prep Homebound (YPHB) forms, which are as follows:

Licensed Physician Condition of Eligibility (YPHB Form 2): This form is to be completed by a
physician and requires answering a series of questions to determine if the student requires HB
services or if his/her medical needs could be met in school through accommodations (i.e. shortened
school days, frequent breaks, etc). We are no longer accepting general physician
statements. Completion of this form is not a guarantee that HB services will be granted. The final
decision will be made by the HB Committee in conjunction with the Manager of Health and Wellness’
professional review.

Statement to Parents (YPHB Form 3): This form outlines parental responsibilities during the General
Education Homebound process. A few highlights include ensuring adult presence during instruction,
providing adequate home accommodations, and providing documentation of (dis)continued medical
needs.

Release/Request for Medical Information (YPHB Form 4): This form authorizes communication
between the physician and school personnel to ensure that accurate information is available for
continuity of student care.

General Education Committee Meeting (YPHB Form 5): This form is the guiding document for the
deciding if the student meet requirements to receive General Education Homebound services. With
the Licensed Physician Condition of Eligibility, student grades, and course outline, the committee
makes a decision about whether services will be provided. A copy of this form is provided to the
parent at the close of the meeting.

General Education Homebound Homework Assignment (YPHB Form 6): This form provides a record
of the assignment and relevant academic attachments provided to the student during homebound
instruction. The teacher should provide a copy of this form to the grade level chair as an account of
instruction provided and potential tutorials needed upon clearance to return to in-school instruction.

General Education Homebound Instruction Log (YPHB 7): This form provides a record of the
instructional days and times. A general education student served at home earns eligible days
present based on the number of hours the student is served at home by a certified teacher. Four
hours of instruction counts as five days of school. Eligible days present are determined each
week. For HB purposes, a week starts Sunday and ends Saturday.

General Education Homebound Exit (YPHB Form 8): This form is to be completed by a physician and
clears the student to return to in-school instruction.
Common HB pitfalls and errors in communication were used to develop a list of Frequently Asked Questions
(FAQs) to further support the process. Student Support Counselors received a training at Content Day,
which included an overview of TEA guidelines and proper facilitation of General Education Homebound
services. The revised protocol is also being shared with School Directors. Feel free to share this information
with your front office team. Should you have any questions and/or needs regarding General Education
