We are looking for organisations that have already started redesigning their overnight support or are ready to start! How will it work? Those who wish to attend must be able to attend all three dates and be able to commit and take an active role during and out with the action learning sets. How do I apply? Overnight Support Action Learning Sets Following the Overnight Support event on the 1st of February, the Improvement Hub (ihub), part of Healthcare Improvement Scotland, the Scottish Government Technology Enabled Care Programme, the Scottish Centre for Telehealth & Telecare and NHS 24 will continue to offer support to Health and Social Care Partnerships and others involved in the redesign of overnight support services. This will be delivered in the form of a series of Action Learning Sets (ALS). Redesigning Overnight Support Services Action Learning Sets Dates and venues: 22nd of June 2017: The Studio, 67 Hope street Glasgow G2 6AE 28th of September2017: venue to be confirmed 25th of January 2018: venue to be confirmed Structure The action learning process will bring peers together in a dynamic and supportive learning environment. It will enable individuals and organisations to accelerate progress towards their goals. Essentially, action learning is about making things happen. At the end of the Action Learning Set, the aim will be for each partnership to have a plan that will enable them to progress to the next phase of their work around overnight supports. Who should attend? HSCPs design ready design that are engaged in the reof overnight support, or are to begin planning their reprocesses. Options for Participating in Action Learning Sets? Action Learning Set 1 The first set will focus on: There are two participation options (see below): reviewing and developing your project plan Option 1: Ready to Go! understanding how you are using data how you are engaging with staff and stakeholders to manage and plan for your change, and reviewing and developing your measurement and evaluation methods to track progress and demonstrate impact. Partnerships who have begun or are ready to begin their re-design work. These partnerships will need a key decision maker leading their work who will actively participate in each session. Partnerships will also require an understanding of their current provision, be able to form a local steering group and have plans to engage with staff, providers and carers. A willingness to share learning and collaborate with others is an essential element of this work. These partnerships will actively participate in the ALS and be supported through their re-design work. Option 2: Partnerships that would like to redesign Overnight Supports but have not yet started planning their approach. These partnerships will be able to send a representative to each ALS and will be able to access learning from a Knowledge Network site. Places We will aim to offer a minimum of two spaces for each participating partnership. Places are limited; therefore we cannot guarantee attendance for all partnerships.
