Research Study Title:

Who can participate? Basic eligibility criteria. (e.g. adults on medication for high
blood pressure; diabetic patients on insulin; normal, healthy adults; etc.)
Where is the study being conducted? Indicate where participants would have to go for
visits.
What do I have to do if I’m in the study? Provide a brief explanation of what you will
ask of the participant.
What is the time commitment for the study? Indicate how much time the participant
would have to commit to enroll in the study (e.g. one thirty minute visit, 4, 8-hour visits
over a year, etc.)
What are the benefits of the study? Indicate what benefits the participant can
reasonably expect to receive.
What will I receive from participating? (If participants will receive
compensation/reimbursements, it can be noted (e.g. reimbursement for parking and/or
your time will be provided.) However, do not overly stress the compensation. In general,
the CCI does not allow dollar values to be specified.
