The Hadley Park & Recreation in conjunction with the Pioneer Valley
Storm is very excited to announce the Fall 2017 soccer program!
Hadley Park and Rec’s soccer program will be entering our teams into the Roots Soccer League. Shaun
Jenks with the Pioneer Valley Storm will be coordinating a fun & structured learning environment for all
kids K-6th grade with professional level coaching (National and International coaching certifications and
vast coaching programs). They will be improving, leading & assisting the volunteer coaches within the
program by providing a clear, proven developmental structure for the towns soccer program that is age
appropriate. We are looking forward to a great new and exciting season. Kindergarten Program (U6)
* Group Size: 12 (Boys & Girls-can be mixed if needed)
* Duration: 8 Weeks
* One 45min Training p/w-Per Week (Saturday mornings)
* Developmental Curriculum provided for one practice per week
* Professional Coach takes all practices, Volunteer Assistant(s) to support
* No Games, No Scrimmage, Just Fun Activities & Trainings
* $70 per player
1st Grade Program (U7)
* Group Size: 12 (4v4)
* Duration: 8 weeks
* Two 1hr Trainings per week
* Fun Drills, Ball Based Curriculum
* 1 Training Developmental, 2nd Training (30min) Drills, (30min) 4v4 Scrimmage
* Developmental Curriculum provided for two practices
* 1-2 Volunteer Coaches to assist and take some leads in 2nd training
* Professional Coach takes team practice on 1st training per week and leads/directs on 2nd w/Volunteer Assistant(s)
* $125 per player
U8 Team: Birthdates 1/1/2009-12/31/2009
* Group Size: 12 (4v4)
* Duration: 9 Weeks
* Two 1hr Trainings per week
* Competitive Games @ Roots Youth Soccer League 4v4 (Weekends-Saturdays)(8 games)
* Developmental Curriculum provided for two practices
* 1-2 Volunteer Coach(s) to assist and take some leads in 2nd
* Professional Coach takes team practice on 1st training per week and leads/directs on 2nd w/Volunteer Assistant(s)
* Uniform provided-game shirt, shorts & game socks…player keeps

Emergency Medical Authorization: In the event that I/We cannot be reached in case of an emergency, I/We authorize any/all medical and/or surgical
treatments, which are deemed advisable by emergency physicians and/or surgeons for my child. I/We also recognize that the patient when admitted is to
remain in the hospital care until his or her physician recommends patient’s discharge. Parent or Guardian also agrees to any photos, which may be taken for Park & Recreation Department use only.
In consideration of the time, training and facilities being made available to my child, I understand and agree that any designated instructor, the Town of
Hadley, and the Park & Recreation Department shall not be liable to me or my child for any claims, actions, suits or harm that may arise out of, or be in
any way related to, my child’s participation in any of the Park & Recreation Programs.

The terms “I”, “me”, “my” also refer to parents or guardians as well as individual participants. In signing this form you are agreeing as follows: I recognize and acknowledge that there are certain risks of physical injury, and I agree to assume the full risk of any injuries, which may be sustained as
a result of my/my child’s normal participation in voluntary extracurricular activities. I further recognize and acknowledge that athletic activities involve
strenuous exertion, potential body contact and risks of injury as a result of normal participation.
I agree to waive and relinquish any and all claims that I may have as a result of normal participation in voluntary extracurricular activities against the
TOWN OF HADLEY, any and all other participating governmental units, any and all independent contractors, officers, agents and employees of the
governmental bodies and independent contractors, and any and all other persons and entities, of whatever nature, that might be directly or indirectly
liable for any injuries that I might sustain from normal participation in the activity (The parties described in the preceding sentence are referred to as
“released parties” in the remainder of the Agreement).
I do hereby fully release and discharge the TOWN OF HADLEY and the other released parties from any and all claims for injuries, damage or loss which
I may have or which may accrue on account of my/my child’s normal participation in voluntary extracurricular activities which do not arise out of or are
not the direct result of negligent, grossly negligent, wanton, willful or intentional acts on the part of the above released parties.
