Selection of the Chapter of the Year will be determined based on the information provided below.
PURPOSE:
The Chapter of the Year Award emphasizes the dissemination and implementation of the current
Clinical Practice Guidelines (Guidelines). This award recognizes a Chapter that demonstrates
initiative and innovation in the implementation of Guidelines’ into practice. The chapter will
receive $1,000.00 and a commemorative plaque that will be presented at the Diabetes
Canada/Canadian Society of Endocrinology & Metabolism (CSEM) Annual Professional
Conference.

Briefly summarize below the Chapter’s activities and achievements regarding the promotion of
the key messages from the Guidelines’ and that have led to their implementation into practice.
Identify both barriers as well as successes of chapter members in meeting the objectives of their
plan.
1. One Chapter meeting will be dedicated to viewing the video presentation of The Essentials
located on the Guidelines website and to discussing the implementation of the 5 Key
Message tools and information in their practice/programs.
a. Copy of meeting agenda and attendance documentation required
2. Define steps to inform and train colleagues on the Guidelines tools for use at point of care
in diabetes education programs and individual practices:
a. Screening and diagnosis of diabetes: (identification of people who have diabetes
and don’t know about it – how do we find them and how do we diagnose them
appropriately?)
b. Blood glucose lowering: (lowering blood sugars reduces the risk of health problems
in the long term and involves both lifestyle and medications – how do we
individualize the choices on how to lower blood sugars based on the “best fit” for
our patients)
c. Cardiovascular risk: (people with diabetes are at higher risk for heart attack and
stroke – how do we help and promote strategies for reducing this risk?)
d. Team care/organization of care: (health-care teams and systems need to be
organized to provide optimal care for the person living with diabetes – how can we
promote and improve this?)
e. Self-management education: (people living with diabetes are in charge of their
chronic disease management – how can the health-care team provide the
knowledge and tools along with the coaching and support needed to empower them
to become good managers of their diabetes?)
