There is no requirement for Minor modifications to be sent to the MAA on a Form 030. New Aircraft or Modifications Classified as Major Submitted PDF versions and versions without ‘Proposed Dates’ at section 5 will not be accepted and your Form 030 will be returned. The individual fields of the application form will automatically grow in size to allow entry of all required information. Please Note that only a Single Platform/Single Task Application request will be accepted per each Form 30 submitted - this is to enable an efficient audit trail to be maintained. Field(s); Project Security Classification: Enter the Project Security Classification as stated in the Defence Policy and Business New Government Security Classification System (GSC) including National Caveats as appropriate as the Form 30 is classed only as Official. 1. The applicant is the TAA or LOAA post holder empowered to endorse DO Certification (Likely to be an SO1/B2 engineering manager). 1.1. Enter your reference (if applicable) – Project File Reference (Form 030 should be maintained as part of the Airworthiness/Engineering file for the type change with the Certification Strategy). 1.2. Enter the full name of the TAA or Project Team (PT) making the application. 1.3. Enter complete registered TAA or Project Team address. 1.4. - 1.6: Enter TAA Lead and Project Certification Lead Names, full telephone number (Inc STD codes), and e-mail of contact persons for this application. (The contact details required for the TAA Lead are the TAA, but this is often but not always the PTL. The Project Certification Lead is the project desk officer responsible for the delivery of the certification work stream. This is the person the MAA Cert Team will treat as the primary POC for MTC progression). 2. Air System Type, Indicate and give a brief description of the Aircraft type, Mk and model applied for (Platform description where details are pertinent to the change being considered or the project is introducing a New Military Type). 2.1. The Engine, Propeller and APU manufacturer and model details:Engine/Propeller/APU Manufacturer – Design Organization and if different OEM, Engine/Propeller/APU Model – Type and Mk or equivalent. 3. For the TAA or the DO under privilege3 to specify the proposed modifications classification iaw RA5810 and RA5820. Major and Minor (modifications) should be identified iaw RA5820, the new Aircraft field should be chosen iaw RA5810. Note: Minor changes are processed under TAA oversight iaw RA5850. There is no requirement for Minor modifications to be sent to the MAA on a Form 030. 3 For the DO scope of privileges refer to RA 5850 – Military Design Approved Organization (MRP 21 Subpart J). A description of the project and identify the systems likely to be affected by the change, and as applicable, make further reference to drawings, basic data, operational characteristics/limitations as required, high level features, technologies ,airworthiness codes. This should reflect the project description within the Airworthiness Strategy. For Modifications this should reflect the “Background” section of the project Certification Strategy which should accompany a Form 030 submission. 3.2. The justification should summarize the nature of the change to the type design and the basis for determining the classification – Attach a separate sheet if required. An explanation of which of the conditions listed in RA5820(1) para 15 has driven the classification to major. An explanation by the TAA or an appropriately approved DO under privilege4 has approved the classification as minor iaw RA5820(3). If certifying a new air system, leave this field blank. 4. For a new type this should contain reference to any existing related Military Type Certificate from other approving authorities, or Civil Type Certificates. For a modification this should contain the Military Type Certificate reference for the Platform and any supporting platform civil type certificates. Where a MTC has not been issued provide the reference to the Platform specification and Certificate of Design and Configuration Status Record. This should also include reference to any existing equipment certification that the modification intends to introduce – Attach a separate sheet if required. 5. The Key Milestone and Critical Path Impact Dates of Type Certification Basis (TCB), Type Certification Exposition (TCE), Type Certification Report (TCR) and the proposed Release to Service (RTS) or intended In Service Date of the platform or change, should be the dates that the project plans to meet ultimate capability delivery requirements in accordance with extant approvals. If these proposed dates are not included on your Form 030, the application will be rejected. If the proposed dates are later changed, this could have an adverse effect on the timely completion of the Military Air Systems Certification Process (RA5810 – Military Type Certificate and RA5820 – Changes in Type Design). Amendment to any of these dates is to be notified to the MAA as soon as possible, using the email address [email protected]. 6.
