Proof of identity framework and requirements Applicants for registration are required to provide one document from each of the following categories (A, B and C, and D if necessary) below: Category A Commencement of identity Category B Born in Australia Full Australian Birth Certificate or Adoption Certificate Australian Passport Immigration status Foreign passport and current Visa Travel document and current Australian Visa Certificate of Evidence of Citizenship Australian Passport (if not already used in category A) Australian drivers licence Link between identity and person by means of photo and signature Category C Evidence of identity operating in community Working with Children check card Foreign passport (if not already used in category A) Firearms or shooters licence Student card Proof of age card Category A or Category B document that has not already been used Change of name certificate Australian Marriage certificate Australian Divorce papers Bank account details/credit card Term deposits notice/Shares certificate PAYG Payment summary with Tax File Number Taxation Assessment notice Health Insurance Card Medicare Card Category D Evidence of identity’s residential address Health Care Card Registration Certificate from a Professional Board Australian Motor Vehicle Registration Pension Card Category B or Category C document which has already been used Rates notice Mortgage papers Lease or tenancy agreement Utility account (gas, water, electricity) Electoral enrolment card The documents provided must meet the following criteria: ALL documents must be true certified or notarised copies of the original At least ONE document must include a recent photograph (SEE CATEGORY B). At least ONE document must be in the applicant’s current name. ALL documents must be officially translated in English. ALL documents must be current/valid at the date of submission. If you have been in Australia less than six weeks If you have been in Australia less than six weeks, you may not be able to produce a Category C or D document. In this circumstance, you will be required to present the following evidence: 1. Original foreign passport or travel document with current Australian visa and proof of date of arrival, and 2. Evidence of current residential address or an original signed statement from the applicant’s prospective employer or sponsor verifying the identity of the applicant. Change of name You must also attach to your application a certified copy of evidence of a change of name if: you have ever been formally known by another name(s), or; any of the documentation you are providing in support of your application is in another name(s) Evidence could include a marriage certificate, decree nisi, deed poll, or Statutory Declaration (which must explain the reason for the discrepancy). The Board has approved a small number of authorised delegates who can sight original documents. The authorised delegates are all located a large distance from a State or Territory AHPRA Office. Please contact your local AHPRA office to inquire whether you can present your documents to an authorised delegate. The Board has approved a small number of authorised delegates who can sight original documents. The authorised delegates are all located a large distance from a State or Territory AHPRA Office. Applicants for general, provisional, specialist and non-practising registration Applicants for general, provisional, specialist and non-practising registration can have their documents for registration certified by an Authorised Officer, in the presence of the applicant. The Authorised Officer should: certify that each document is a true and correct copy of the original, and certify that the photograph on photographic documentation (e.g. licence or passport) is a true likeness of the applicant, and witness the signature of the applicant. Authorised certifiers are: Justice of the Peace, Bail Justice, Registrar or Deputy Registrar Commissioner for Declarations, Commissioner for Oaths, Commissioner for Affidavits (dependent on jurisdictions) Bank manager or employee with 5 or more years of continuous service Senior State or Commonwealth public service employee Minister of Religion authorised to celebrate marriages Member of Commonwealth or State Parliament Australian lawyer Accountant (member of the ICA, ASA, NIA or CPA) Registered members of the teaching profession Councillor of a Municipality Secretary of a building society Member of the police force Sheriff or Deputy Sheriff Public Notary Clerk to an Australian lawyer Medical Board of Australia Proof of Identity framework and requirements Approved on 24 March 2010 Page 4 of 5 Outside Australia, documents will be accepted that are certified by a Notary Public and persons appointed to hold, or act in, the office in a country/place outside Australia in an Australian Embassy, High Commission or Legation as: Australian Consul-General, Consul or Vice-Consul Australian Trade Commissioner of Consular Agent Australian Ambassador or High Commissioner Australian Minister, Head of Mission, Commissioner, Charge d’affaires or Counsellor Australian Secretary or Attache.
