POSITION DETAILS 3.1 Position Context The services and programs of ISCH are currently grouped by target group or work area. The Community Care Program consists of staff from various disciplines, located across four centres. The target groups are primarily the frail aged, people with disabilities and their carers, and include people from culturally and linguistically diverse backgrounds. Services provided include community nursing, Indigenous access, social work, physiotherapy, podiatry, dietetics, speech pathology and occupational therapy, with a range of special projects and group activities. This position is funded by the Department of Human Services and aims to assist people with an ABI over 18 years of age to maintain independent, stable housing. The position is supported within a dynamic and supportive team, and receives strong clinical supervision. 3.2 Position Objective The ABI Case Manager will provide flexible, comprehensive Case Management and supportive outreach services to adults aged 18+ with an Acquired Brain Injury (non compensable), whose needs are impacting upon their ability to access or maintain public housing. This program includes people residing within the Cities of Port Phillip, Stonnington, and Glen Eira. Internal / Confidential © Inner South Community Health Review Date: 27.11.14 Reviewed by: GH Page 2 of 7 Position Description The ABI Case Manager, as a member of the Community Care Program, will also have a small generalist Case Management load (0.2 EFT) as part of their 0.9 EFT position. This target group is primarily people over the age of 65, their carers and includes people from culturally and linguistically diverse backgrounds. Many clients have multiple and complex needs. The ABI Case Manager will play a role in the ongoing development and resourcing of the team and the program in working with ABI clients and they will, at times, work closely with staff members of other programs on client or organisational issues. The worker will contribute to advocacy and the development of the service system to meet the needs of the target group. 3.3 Key Responsibilities Client Services Provide comprehensive and flexible case management services to the target group. Provide assertive outreach to support socially isolated clients. Provide referral, as appropriate, to services within ISCH, as well as to other health and community services. Provide direct support services where appropriate, for example, assisting clients to appointments. Arrange the expenditure of Flexible Care Funds for ABI clients requiring support services as required Participate in various quality assurance processes, including case allocation, case review, and support and supervision as required. Maintain case files, documentation and statistics according to ISCH policy. Program and Service Development Contribute to the ongoing development and evaluation of the ABI Housing Support position Develop and maintain strong links with other Community Care services, the Community Connections Program and other ISCH services as appropriate, the Office of Housing and other housing services. Act as a resource within the organisation regarding Acquired Brain Injury. Facilitate the development of joint program initiatives with other service providers, as appropriate. Contribute to the ongoing development of the Community Care Program by participating in planning and reviewing services and programs. Contribute to ISCH policy development, working groups and specific task groups as required. Participate in, and contribute to, Centre activities which foster the development of the local service profile and service delivery system. KEY SELECTION CRITERIA (Must be submitted with your Application) Mandatory Degree in Social Work Experience working with Acquired Brain Injury clients Demonstrated skills in networking, liaison and the development and maintenance of strong links with relevant service providers and users. An understanding of and commitment to the target group Demonstrated knowledge of the aged and disability service system and the public housing sector. Demonstrated ability to provide outreach Case Management services to people with complex needs who are socially isolated and/or have challenging behaviours. Demonstrated ability to contribute to the functioning of a multi-disciplinary team. Well-developed interpersonal and communication skills. Demonstrated understanding of working with people from linguistically and culturally diverse backgrounds. Current Victorian Driver’s licence. Applicants must have registered for A Working With Children Check to be eligible for this position. (if relevant to this role) Desirable Proficiency in a community language other than English. Experience working with and influencing the service system to improve responsiveness to the client group. CONDITIONS OF EMPLOYMENT Social, Community Homecare and disability Services (SCHADS) Award: (depending on qualification) Salary Level: SCHADS Level 5 Hours Per Week: 68.4 (0.9 EFT) Duration: Ongoing Probationary Period: In accordance with our usual practice, there will be a probationary period of three months, during which time a review process will be undertaken. At the expiration of the probationary period, employment may be confirmed as an ongoing employee, employment may be terminated or the probationary period may be extended up to a maximum of 6 months with further review. During the probationary period either party can terminate for any reason. Location: Prahran site From time to time the incumbent may be requested to work from or be based at other ISCH centres. Salary Packaging Information: Salary packaging is a legitimate method of restructuring existing salary into a combination of salary and expense payment benefits in order to provide a higher net remuneration for the employee. This offer applies while Inner South Community Health Service continues to be granted "public benevolent institution" status and enjoys exemption from the provisions of the Fringe Benefits Tax (FBT). Should our status change salary packaging will no longer be available. We offer 3 types of Salary Packaging:o Standard tax free salary packaging amount $15,900 [per annum o Meal Entertainment Expenses $5,000 per annum o Entertainment Facility Leasing Expenses - no threshold per annum It is not compulsory for employees to take up the offer to ‘package’ their salary; however the benefits from packaging are considerable.
