130744, Birmingham AL 35203. Teams with entry fees must be submitted by Tuesday,
April 1, 2014.

Liability Release (attached) must be signed and submitted by each team member on the
day of the benefit. Blank release forms will be available at registration, or downloaded
via the link above to complete and submit during benefit registration.

T-shirts will be provided the morning of the tournament to the first 250 players, so sign
up early.

All entrants must be 21 or older.
Teams shall consist of four players and one designated driver which may play each
hole if desired.
Designated Drivers will not be permitted to consume alcoholic beverages during the
benefit, but will receive soft drinks from participating businesses, a T-shirt, and be
eligible for any prizes their team wins. Should the Designated Driver consume any
alcoholic beverage, the team will be disqualified immediately.
Designated Drivers are responsible for transporting his or her team from “hole to
hole” AND TRANSPORTATION OF THE TEAM HOME FOLLOWING THE
CONCLUSION OF CLOSING AWARDS PROGRAM.
Entry fee is $200.00 per team with all proceeds benefiting The Exceptional
Foundation in Homewood and projects of the ACTION Civitan Club to assist people
with disabilities and underprivileged children/families in our area.
The “course” shall consist of local businesses. Each establishment shall design and
build its own “hole.” The course shall consist of 9 holes (short course). A listing of
holes and a map indicating locations will be provided during team registration.
Each team is responsible for presenting their scorecard to the Onsite Judge at each
hole and submitting their cards promptly at the end of play. The team score will be
the lowest player’s score recorded for each hole. Your scorecard should have only
the lowest score per hole listed. Onsite judges MUST record score; not players.
Players must supply their own putter and ball.
Each team is limited to 15 minutes per hole. Please proceed to the next hole
promptly to keep the benefit running smoothly.
The first 50 teams (250 players) to sign up (and pay!) will receive player T-shirts!
Along with the 1st, 2nd, and 3rd place team prizes, there will be a nice prize for the best
(or worst) dressed team. All awards and prizes will be presented during the Closing
Awards Program at Good People Brewing Company as soon as all teams are in and
scored.
Registration at Good People Brewing Company, 114 14th St S, Birmingham, AL
35233 begins at 1:00 pm Saturday, April 5. Judges will arrive at course locations by
1:30 pm. Play will begin promptly at 2:00 pm. All team members must register at
Good People Brewing Company to receive a scorecard, course map with locations
and ask any questions.
The onsite judge will rule on any questionable shots. Judge’s decision is FINAL.
ONLY Judges will record scores on scorecards.
In the case of a tie on total scores, a putt-off will be held during the Closing Awards
Program. One player from each team will putt-off one hole.

All teams will submit completed scorecards immediately to Head Scorekeeper at
Good People Brewing Company after the team’s final round. Failure to submit
completed scorecards in a timely manner may disqualify team.
NOTE: Drinks cannot be carried from bar to bar.
ACTION CIVITAN CLUB PUTT-PUTT GOLF BENEFIT - LIABILITY RELEASE
In consideration of being permitted to participate in the ACTION Civitan Putt-Putt/Pub Crawl
Golf Benefit to be held on the 5th day of April, 2014, beginning at 1:00 p.m., I the undersigned,
for myself, successors, heirs and assigns, Release and Forever Discharge The Exceptional
Foundation, ACTION Civitan, and all other sponsors, their affiliates, subsidiaries, agents,
employees, servants, officers, and directors (hereinafter collectively referred to as “ACTION”)
from all claims, causes of action or judgments I may have or claim to have against ACTION for
all personal injuries, including death, and injuries to property, real or personal, caused by or
arising out of my participation in the ACTION Civitan Club Putt-Putt/Pub Crawl Golf Benefit.
I further agree for myself, successors, heirs and assigns to indemnify and hold ACTION
harmless from all claims and suits for personal injuries, including death, and damages to
property caused by act or omission arising out of my participation in the ACTION Civitan PuttPutt/Pub Crawl Golf Benefit, and from all judgments recovered and from all expenses incurred
in defending said claims of suits.
I further agree that my photographs, pictures, slides, and or movies taken or made by ACTION
in connection with my participating in the ACTION Civitan Club Putt-Putt/Pub Crawl Golf Benefit,
or any reproduction of the same, as well as my name, may in any manner be used by ACTION
or by any person, corporation or association authorized by ACTION. I am in good health and
have no physical condition that would prevent me from participating in this event.
I further agree that if I violate any of the rules, procedures, or regulations of the ACTION Civitan
Club Putt-Putt/Pub Crawl Golf Benefit, which are attached hereto, I agree that my participation
in this event will be terminated and I may be requested to leave any premises where this event
may take place.
